郑氏“颈四步”手法辅助微针刀治疗神经根型颈椎病

摘　　要：目的探究郑氏“颈四步”手法辅助微针刀治疗神经根型颈椎病的临床治疗效果。方法前瞻性随机选取本院在2020年5月-2023年5月间收治的80例神经根型颈椎病患者纳入研究,将其用随机数字法分成对照组和观察组,各40例。对照组予以微针刀治疗,观察组予以郑氏“颈四步”手法辅助微针刀治疗。比较2组临床疗效、中医症状、疼痛改善、颈椎活动度、血液流变学水平。结果观察组总有效率95.00%高于对照组75.00%(P<0.05)。治疗后,观察组颈项疼痛、颈项拘挛、上肢疼痛、活动不利、手指麻木评分低于对照组(P<0.05)。观察组治疗7 d后、14 d后视觉模拟评分法(VAS)评分低于对照组(P<0.05)。观察组左旋转、右旋转、左侧屈、右侧屈活动度均高于对照组(P<0.05)。观察组血浆黏度、全血高切黏度、全血低切黏度均低于对照组(P<0.05)。结论对神经根型颈椎病患者予以郑氏“颈四步”手法辅助微针刀治疗,可明显提升疗效、改善症状,同时有效减轻疼痛、提升颈椎活动度、改善血液流变,促进颈椎功能恢复。Objective To explore the clinical effect of Zheng's"four manipulations for cervical spondylosis treatment"assisting micro-acupotomy therapy in the treatment of cervical spondylotic radiculopathy(CSR).Methods Eighty patients with CSR admitted to our hospital from May 2020 to May 2023 were prospectively and randomly selected for the study.They were divided into two groups by random number method,with 40 cases in each group.The control group was treated with the micro-acupotomy therapy,while the observation group was treated with Zheng's"four manipulations for cervical spondylosis treatment"to assist the micro-acupotomy therapy.The clinical efficacy,levels of TCM symptoms,pain improvement,cervical motion,and hemorheology were compared between the two groups.Results The total effective rate of the observation group was 95.00%,which was higher than 75.00%of the control group(P<0.05).After treatment,the scores of neck pain,neck spasm,upper limb pain,poor mobility and finger numbness were lower in the observation group than those in the control group(P<0.05).After 7 days and 14 days,the visual analogue scale(VAS)scores of the observation group were lower than those of the control group(P<0.05).In the observation group,the scores of left rotation,right rotation,left flexion,right flexion were higher than those in the control group(P<0.05).The levels of plasma viscosity,whole blood high shear viscosity,whole blood low shear viscosity were lower in the observation group than those in the control group(P<0.05).Conclusion Zheng's"four manipulations for cervical spondylosis treatment"assisting the micro-acupotomy therapy can significantly improve the curative effect and symptoms in patients with CSR.At the same time,it can effectively reduce pain,improve the motion of cervical spine,and improve blood bleeding,promoting the recovery of cervical spine function.
