虚寒胃痛颗粒联合西药治疗老年幽门螺杆菌阳性胃溃疡临床研究

摘　　要：目的:观察虚寒胃痛颗粒联合西药治疗老年幽门螺杆菌(Hp)阳性胃溃疡(GU)的临床疗效及对炎症因子的影响。方法:采用随机数字表法将126例老年Hp阳性GU患者分为治疗组与对照组各63例。对照组给予常规西药治疗,治疗组在对照组基础上加用虚寒胃痛颗粒治疗。比较2组治疗前后中医证候积分、胃肠激素指标、炎症因子水平;比较2组临床疗效、Hp清除率及不良反应发生率。结果:治疗后,治疗组总有效率、Hp清除率分别为92.06%、90.48%,高于对照组79.37%、76.19%(P<0.05)。2组治疗后中医证候积分及血清胃泌素(GAS)、白细胞介素-6 (IL-6)、白细胞介素-8 (IL-8)、肿瘤坏死因子-α (TNF-α)水平均较治疗前降低(P<0.05),血清胃动素(MTL)水平升高(P<0.05);治疗组治疗后中医证候积分及血清GAS、IL-6、IL-8、TNF-α水平低于对照组(P<0.05),血清MTL水平高于对照组(P<0.05)。治疗组不良反应发生率4.76%,低于对照组15.87%(P<0.05)。结论:虚寒胃痛颗粒联合西药治疗老年Hp阳性GU疗效确切,能改善患者胃肠激素水平,减少炎症因子分泌与Hp感染,安全性较高。Objective:To observe the clinical effect of Xuhan Weitong Granules combined with western medicine on senile patients with helicobacter pylori(Hp)positive gastric ulcer(GU)and its effect on inflammatory factors.Methods:A total of 126 cases of senile patients with Hp-positive GU were divided into the treatment group and the control group according to the random number table method,with 63 cases in each group.The control group was treated with routine western medicine,and the treatment group was additionally treated with Xuhan Weitong Granules based on the treatment of the control group.Traditional Chinese medicine(TCM)syndrome scores,gastrointestinal hormone indexes and inflammatory factors before and after treatment were compared between the two groups:clinical effects,Hp clearance rates and the incidence of adverse reactions were compared between the two groups.Results:After treatment,the total effective rate and Hp clearance rate were 92.06%and 90.48%respectively in the treatment group,higher than those of 79.37%and 76.19%respectively in the control group(P<0.05).After treatment,TCM syndrome scores and the levels of gastrin(GAS),interleukin-6(IL-6),interleukin-8(IL-8)and tumor necrosis factor-α(TNF-α)in serum in the two groups were decreased when compared with those before treatment(P<0.05),and the levels of serum motilin(MTL)were increased(P<0.05):TCM syndrome scores and the levels of GAS,IL-6,IL-8 and TNF-αin serum in the treatment group were lower than those in the control group(P<0.05),the level of serum MTL was higher than that in the control group(P<0.05).The incidence of adverse reactions was 4.76%in the treatment group,lower than that of 15.87%in the control group(P<0.05).Conclusion:Xuhan Weitong Granules combined with western medicine has a definite curative effect on Hp-positive GU in senile patients.It can improve the levels of gastrointestinal hormones,and reduce the secretion of inflammatory factors and Hp infection,With high safety.
