不同冠根比种植单冠修复体对口腔种植患者治疗效果的影响

摘　　要：目的 分析不同冠根比种植单冠修复体对口腔种植患者治疗效果的影响。方法 筛选行口腔种植的患者98例(98枚种植体)作为研究对象,按照双色球法分为研究组及对照组,各49例(49枚)。对照组采用冠根比<1的种植单冠修复体,研究组采用冠根比1~2的种植单冠修复体。比较两组种植后1、6、18个月种植体脱落发生率及种植体周边骨吸收情况,治疗前及治疗后30、90、180 d龈沟液骨代谢情况(包括碱性磷酸酶、骨保护素、骨钙素)。结果 研究组种植后1、6、18个月的种植体脱落发生率分别为2.04%、0、2.04%,对照组种植后1、6、18个月的种植体脱落发生率分别为0、2.04%、4.08%。两组患者种植后1、6、18个月的种植体脱落发生率相差无异,无统计意义(P>0.05)。研究组患者种植后1个月种植体周边骨吸收量为(0.42±0.12)mm、种植后6个月种植体周边骨吸收量为(0.52±0.13)mm、种植后18个月种植体周边骨吸收量为(0.83±0.10)mm。对照组患者种植后1个月种植体周边骨吸收量为(0.40±0.11)mm、种植后6个月种植体周边骨吸收量为(0.54±0.12)mm、种植后18个月种植体周边骨吸收量为(0.84±0.14)mm。两组患者种植后1、6、18个月种植体周边骨吸收量相差无异,无统计意义(P>0.05)。两组患者治疗前及治疗后30、90、180 d的龈沟液碱性磷酸酶、骨保护素、骨钙素水平相差无异,无统计意义(P>0.05)。结论 冠根比<1,或冠根比1~2的种植单冠修复体对口腔种植患者均具有良好的治疗效果,均值得推广利用。Objective To investigate the effect of implanting single-crown prosthesis with different crown-root ratios in the treatment of oral implant patients.Methods 98 patients(98 implants)with single crown restoration as study subjects were divided into a study group and a control group according to bicolor sphere method,with 49 cases(49 implants)in each group.The control group was implanted with single-crown prosthesis with crown-root ratio<1,and the study group was implanted with single-crown prosthesis with crown-root ratio of 1-2.The incidence of dental implant detachment,bone resorption around the implant at 1,6 and 18 months after implantation,and the bone metabolism(including alkaline phosphatase,osteoprotegerin and osteocalcin)in gingival crevicular fluid before treatment and 30,90 and 180 d after treatment were compared between the two groups.Results The incidence of dental implant detachment at 1,6 and 18 months after implantation in the study group were 2.04%,0 and 2.04%,while those in the control group were 0,2.04%and 4.08%.There was no statistical significance in the incidence of dental implant detachment at 1,6 and 18 months after implantation between the two groups(P>0.05).In the study group,the bone resorption around the implant was(0.42±0.12)mm at 1 month after implantation,(0.52±0.13)mm at 6 months after implantation,and(0.83±0.10)mm at 18 months after implantation;in the control group,the bone resorption around the implant was(0.40±0.11)mm at 1 month after implantation,(0.54±0.12)mm at 6 months after implantation,and(0.84±0.14)mm at 18 months after implantation.There was no significant difference in bone resorption around the implant between the two groups at 1,6 and 18 months after implantation(P>0.05).The levels of alkaline phosphatase,osteoprotegerin and osteocalcin in gingival crevicular fluid were compared between the two groups before treatment and 30,90 and 180 d after treatment,and the difference was not statistically significant(P>0.05).Conclusion Single-crown prosthesis with a crown-ro
