甲状腺被膜解剖联合环甲隙显露喉返神经法在甲状腺癌全切术患者中的应用分析

摘　　要：目的 探讨甲状腺被膜解剖联合环甲隙显露喉返神经法在甲状腺癌(TC)全切术患者中的应用效果。方法 选取2020年1月~2022年4月本院80例TC全切术患者作为研究对象,按照随机数字表将其分为两组,即对照组(40例)与研究组(40例)。两组均行TC根治术,对照组采用常规甲状腺下动脉显露喉返神经法进行手术,研究组采用甲状腺被膜解剖联合环甲隙显露喉返神经法进行手术。观察并比较两组手术时间与淋巴结清除数量、血清钙离子(Ca2+)及甲状旁腺素(PTH)水平,以及喉返神经与甲状旁腺的损伤情况。结果 两组手术时间与淋巴结清除数量比较,结果未见差异性(P>0.05)。研究组术后7d时Ca2+及PTH水平较对照组高(P<0.05)。研究组喉返神经与甲状旁腺的暂时性损伤发生率较对照组低(P<0.05)。结论 TC全切术中采用甲状腺被膜解剖联合环甲隙显露喉返神经法可以有效保护患者喉返神经与甲状旁腺功能,适于临床推广。Objective To investigate the effect of thyroid capsule anatomy combined with exposure of recurrent laryngeal nerve through the cricoid thyroid space in patients with thyroid cancer(TC)undergoing total resection.Methods From January 2020 to April 2022,80 patients with total TC resection in our hospital were randomly divided into control group(40 cases)and study group(40 cases).In the control group,the recurrent laryngeal nerve was exposed by the routine inferior thyroid artery,while in the study group,the recurrent laryngeal nerve was exposed by the thyroid capsule anatomy combined with the cricoid thyroid space.Results There was no difference in the operation time and the number of lymph nodes removed between the two groups(P>0.05).The levels of Ca2+and PTH in the study group were higher than those in the control group 7 days after operation(P<0.05).The incidence of temporary injury of recurrent laryngeal nerve and parathyroid gland in the study group was lower than that in the control group(P<0.05).Conclusion The method of thyroidectomy combined with thyroidectomy to expose recurrent laryngeal nerve can effectively protect the function of recurrent laryngeal nerve and parathyroid gland,which is suitable for clinical promotion.
