摘 要:目的:探讨微小RNA-155(miR-155)、程序性细胞死亡因子4(PDCD4)在炎症性肠病(IBD)患者血清、肠黏膜组织中的表达水平及其相关性。方法:选取2018年11月—2022年3月克罗恩病(CD)患者78例为CD组,78例溃疡性结肠炎(UC)患者为UC组,另选取同期行肠镜检查的50例结肠息肉患者为对照组。比较对照组、CD组、UC组血清、肠黏膜组织miR-155、PDCD4 mRNA表达水平;按简化CD活动指数(CDAI)将CD患者分为轻度CD活动组(24例)、中度CD活动组(28例)、重度CD活动组(26例),依据Mayo评分将UC患者分为轻度UC活动组(23例)、中度UC活动组(30例)、重度UC活动组(25例),比较不同活动度CD与UC患者血清、肠黏膜组织miR-155、PDCD4 mRNA表达水平;分析CD、UC患者血清、肠黏膜组织中miR-155表达水平与PDCD4 mRNA的关系。结果:CD组、UC组患者血清、肠黏膜组织miR-155表达水平高于对照组(P<0.05),PDCD4 mRNA表达水平低于对照组(P<0.05);随CD/UC活动度增加,CD/UC患者血清、肠黏膜组织miR-155表达水平逐渐升高(P<0.05),PDCD4 mRNA表达水平逐渐降低(P<0.05);CD患者血清、肠黏膜组织中miR-155表达水平与PDCD4 mRNA均呈负相关(P<0.05);UC患者血清、肠黏膜组织中miR-155表达水平与PDCD4 mRNA均呈负相关(P<0.05)。结论:IBD患者血清、肠黏膜组织中miR-155表达上调,PDCD4 mRNA表达下调,miR-155与PDCD4 mRNA呈负相关,且miR-155、PDCD4 mRNA可能是临床评估CD、UC严重程度的辅助指标。Objective:To investigate the expression levels of microRNA⁃155(miR⁃155)and programmed cell death factor 4(PDCD4)in serum and intestinal mucosa of patients with inflammatory bowel dis⁃ease(IBD)and the correlation between miR⁃155 and PDCD4.Methods:A total of seventy⁃eight Crohn's disease(CD)patients admitted from November 2018 to March 2022 were taken as the CD group,and 78 ulcerative colitis(UC)patients were taken as the UC group.Another 50 patients with colon polyps who underwent colonoscopy were taken as the control group.The expression levels of miR⁃155 and PDCD4 mRNA in serum and intestinal mucosa tissue of the control group,CD group,and UC group were compared;according to the simplified CD activity index(CDAI),CD patients were grouped into mild CD activity group(24 cases),moderate CD activity group(28 cases),and se⁃vere CD activity group(26 cases).UC patients were grouped into mild UC activity group(23 cases),moderate UC activity group(30 cases),and severe UC activity group(25 cases)according to the Mayo score.The expression levels of miR⁃155 and PDCD4 mRNA in serum and intestinal mucosa of patients with different activity levels of CD/UC were compared;the relationship between the expres⁃sion level of miR⁃155 and PDCD4 mRNA in serum and intestinal mucosa of CD and UC patients was analyzed.Results:The expression levels of miR⁃155 in serum and intestinal mucosa of patients in CD group and UC group were higher than those in the control group(P<0.05),and the expression level of PDCD4 mRNA was lower than that in the control group(P<0.05);with the increase of CD/UC activity,the expression level of miR⁃155 in serum and intestinal mucosa of CD/UC patients gradually increased(P<0.05),while the expression level of PDCD4 mRNA gradually decreased(P<0.05);the expression level of miR⁃155 in serum and intestinal mucosa of CD patients was negatively correlated with PDCD4 mRNA(P<0.05);the expression level of miR⁃155 in serum and intestinal mucosa of UC patients was negatively correlated with PDCD4
