HIF-1α、cyclin-D1在膀胱癌组织中的表达及其临床意义分析

摘　　要：目的分析缺氧诱导因子1α(HIF-1α)、细胞周期蛋白D1(cyclin-D1)在膀胱癌组织中的表达及其临床意义。方法收集2015年3月—2020年6月该院行手术治疗的146例膀胱癌患者膀胱癌组织和癌旁组织样本,采用免疫组织化学法检测HIF-1α和cyclin-D1表达情况,分析HIF-1α和cyclin-D1表达水平与临床病理的关系,分析膀胱癌患者预后情况,比较不同预后患者癌组织HIF-1α和cyclin-D1表达水平及关系。结果膀胱癌组织中HIF-1α和cyclin-D1阳性率和表达评分均显著高于癌旁组织(P<0.05);肿瘤直径≥3cm的HIF-1α和cyclin-D1表达水平显著高于肿瘤直径<3cm(P<0.05),临床分期T3~T4的HIF-1α和cyclin-D1表达水平显著高于临床分期T1~T2(P<0.05),病理分级中,HIF-1α和cyclin-D1表达水平G3>G2>G1(P<0.05),淋巴转移的HIF-1α和cyclin-D1表达水平显著高于未转移(P<0.05);随访至2023年6月,146例患者出现59例复发(40.41%),87例未复发(59.59%);146例患者存活134例(91.78%),死亡12例(8.22%);复发组HIF-1α和cyclin-D1表达水平显著高于未复发组(P<0.05),死亡组HIF-1α和cyclin-D1表达水平显著高于存活组(P<0.05);HIF-1α和cyclin-D1低表达患者无病生存期显著长于高表达患者(logrankχ^(2)=4.237,log-rankχ^(2)=6.525,P<0.05)。结论HIF-1α和cyclin-D1表达水平与膀胱癌病情进展及生存预后相关,可作为临床诊治参考指标。Objective To analyze the expressions and clinical significance of hypoxia-inducible factor 1α(HIF-1α)and cyclin-D1 in bladder cancer tissues.Methods Samples of bladder cancer tissues and adjacent bladder tissues of one hundred and forty-six patients with bladder cancer undergoing surgical treatment in Neijiang Second People's Hospital from March 2015 to June 2020 were collected.The expressions of HIF-1αand cyclin-D1 were detected by immunohistochemistry,and the relationship between expression levels of HIF-1αand cyclin-D1 and clinicopathology was analyzed.The prognosis of patients with bladder cancer was analyzed and levels of HIF-1αand cyclin-D1 in cancer tissues of patients with different prognosis status were compared.Results The positive rates and expression scores of HIF-1αand cyclin-D1 in bladder cancer tissues were significantly higher than that in adjacent tissues(P<0.05).The expression levels of HIF-1αand cyclin-D1 in patients with tumor diameter≥3cm were significantly higher than that in patients with tumor diameter<3cm(P<0.05).HIF-1αand cyclin-D1 in patients with clinical stage T3-T4 were significantly higher than that in patients with clinical stage T1-T2(P<0.05).According to pathological grading,HIF-1αand cyclin-D1 were manifested as G3>G2>G1(P<0.05).HIF-1αand cyclin-D1 in patients with lymphatic metastasis were significantly higher than that without metastasis(P<0.05).Among 146 patients who were followed up to June 2023,59 cases recurred(40.41%)and 87 cases did not recur(59.59%).134 cases(91.78%)survived and 12 cases(8.22%)died among 146 patients.HIF-1αand cyclin-D1 in recurrence group were significantly higher than that in non-recurrence group(P<0.05),and the levels in death group were significantly higher than that in survival group(P<0.05).The disease-free survival time of patients with low HIF-1αand cyclin-D1expressions was significantly longer than that in patients with high expressions(log-rankχ^(2)=4.237,log-rankχ^(2)=6.525,P<0.05).Conclusion HIF-1αand cyclin-D1 are correlate
