白头翁汤加味联合美沙拉嗪肠溶片用于治疗湿热型溃疡性结肠炎的疗效观察

摘　　要：目的:观察白头翁汤加味联合美沙拉嗪肠溶片用于治疗湿热型溃疡性结肠炎的疗效。方法:选取医院2021年3月~2022年5月就诊的湿热型溃疡性结肠炎患者102例,以随机数字表法将其分成观察组(n=51,在对照组的基础上接受白头翁汤治疗)及对照组(n=51,采用美沙拉嗪肠溶片治疗)。对照组接受美沙拉嗪肠溶片治疗,观察组在对照组的基础上接受白头翁汤加味治疗,两组均治疗4周。对比两组中医证候积分、改良Mayo评分系统、炎症性肠病生活质量问卷(IBDQ)、基质金属蛋白酶-1(MMP-1)与白细胞介素-1β(IL-β)、二胺氧化酶(DAO)、D-乳酸(D-LA)、用药安全性及临床疗效。结果:治疗后,两组中医证候积分均降低(P<0.05),且观察组比对照组低(P<0.05)。两组治疗后的改良Mayo评分均降低(P<0.05),且观察组更低(P<0.05);治疗后,两组IBDQ评分均升高(P<0.05),且观察组更高(P<0.05)。治疗后,两组血清MMP-1、IL-1β水平均降低(P<0.05),且观察组更低(P<0.05)。治疗后,两组血清DAO、D-LA水平均降低(P<0.05),且观察组更低(P<0.05)。两组总不良反应发生率对比无显著性差异(P>0.05)。观察组的总有效率(94.12%)比对照组(80.39%)高(P<0.05)。结论:白头翁汤加味联合美沙拉嗪肠溶片用于治疗湿热型溃疡性结肠炎疗效确切,可减轻患者炎症反应,改善生活质量与肠道屏障功能,且不会显著增加药物不良反应。Objective:To observe the curative effect of Baitouweng decoction combined with mesalazine enteric-coated tablets on the treatment of damp-heat ulcerative colitis.Methods:Clinical data of 102patients with damp-heat ulcerative colitis admitted to the hospital from March 2021to May 2022were retrospectively analyzed,and they were randomly divided into observation group(n=51,which was treated with Baitouweng decoction on the basis of control group)and control group(n=51,which was treated with mesalazine enteric-coated tablets)by the method of random number table.Both groups were treated for 4weeks.Traditional Chinese medicine syndrome score,modified Mayo score system,Inflammatory Bowel disease Quality of Life Questionnaire(IBDQ),matrix metalloproteinase-1(MMP-1)and interleukin-1β(IL-β),diamine oxidase(DAO),D-lactic acid(DLA),drug safety and clinical efficacy were compared between the two groups.Results:After treatment,the TCM syndrome score of both groups was decreased(P<0.05),and the observation group was lower than control group(P<0.05).The modified Mayo score after treatment was decreased in both groups(P<0.05),and was lower in the observation group(P<0.05).After treatment,IBDQ score was increased in both groups(P<0.05),and higher in observation group(P<0.05).After treatment,serum levels of MMP-1and IL-1βwere decreased in both groups(P<0.05),and lower in observation group(P<0.05).After treatment,serum DAO and D-LA levels were decreased in both groups(P<0.05),and lower in observation group(P<0.05).In the two groups,there was no difference in the incidence of total adverse reactions(P>0.05).The observation group of total effective rate(94.12%)was higher than control group(80.39%)(P<0.05).Conclusions:Pulsatilla decoction combined with mesalazine enteric-coated tablets is effective in the treatment of damp-heat ulcerative colitis,which can reduce the inflammatory response of patients,improve the quality of life and intestinal barrier function,and do not significantly increase the adverse drug reactions.
