摘　　要：目的探讨根管填充联合中药治疗牙周-牙髓联合病变的临床疗效。方法选取2021年4月至2022年7月新余市中医院收治的90例牙周-牙髓联合病变患者作为研究对象,采用随机数字表法将其分为对照组(45例)、观察组(45例);对照组采用根管填充治疗,观察组采用根管填充联合中药治疗;比较两组的疗效、疼痛程度、炎症因子水平及牙周临床指标。结果观察组患者的总有效率高于对照组,差异有统计学意义(P<0.05);治疗后3个月,观察组患者的视觉模拟评分法(VAS)评分、白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平均低于对照组,差异有统计学意义(P<0.05);治疗后6个月,观察组患者的菌斑指数(PLI)、牙龈指数(GI)评分均较对照组低,差异有统计学意义(P<0.05)。结论根管填充联合中药治疗牙周—牙髓联合病变效果较好,可减轻疼痛程度,降低炎症因子水平,改善牙周情况。Objective To investigate the clinical effect of root canal filling combined with traditional Chinese medicine in the treatment of periodontal and pulp lesions.Methods A total of 90 patients with combined periodontal and pulp lesions admitted to Xinyu Hospital of Traditional Chinese Medicine from April 2021 to July 2022 were selected as the study objects,and they were divided into control group(45 cases)and observation group(45 cases)according to random number table method.The control group was treated with root canal filling,and the observation group was treated with root canal filling combined with Chinese medicine.The therapeutic effect,pain degree,inflammatory factor level and periodontal clinical indexes were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).Visual analogue scale(VAS)score,interleukin-1β(IL-1β),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)levels in observation group were lower than those in control group at 3 months after treatment,with statistical significances(P<0.05).The plaque index(PLI)and gingival index(GI)scores of the observation group were lower than those of the control group at 6 months after treatment,and the differences were statistically significant(P<0.05).Conclusion The effect of root canal filling combined with traditional Chinese medicine in the treatment of periodontal and pulp lesions is better,which can reduce the degree of pain,reduce the level of inflammatory factors and improve the periodontal condition.
