摘 要:目的分析血清鳞状细胞癌相关抗原(SCC-Ag)、人附睾分泌蛋白4(HE4)、SMAD4水平与宫颈癌放化疗敏感性的相关性。方法选取接受放化疗治疗的宫颈癌患者74例为研究对象,根据放化疗敏感性分为抵抗组和敏感组,分别有48例和26例。检测患者治疗前后血清SCC-Ag、HE4及SMAD4 mRNA水平,分析治疗前血清SCC-Ag、HE4、SMAD4水平与超声参数的相关性。绘制受试者工作特征曲线(ROC)描述治疗前SCC-Ag、HE4及SMAD4 mRNA水平对放化疗敏感性的预测价值。结果治疗后2组患者血清SCC-Ag、HE4水平较治疗前降低,SMAD4 mRNA水平较治疗前升高(P<0.05);且敏感组患者治疗前、后SCC-Ag、HE4水平均低于抵抗组,SMAD4 mRNA水平均高于抵抗组(P<0.05)。治疗前血清SCC-Ag、HE4与VI呈正相关(γ=0.518、0.544,P<0.05),SMAD4 mRNA与VI呈负相关(γ=-0.581,P<0.05)。ROC曲线显示,治疗前SCC-Ag、HE4及SMAD4 mRNA联合对放化疗敏感性的预测价值较高,AUC为0.863(95%CI:0.764~0.962)。结论血清SCC-Ag、HE4及SMAD4 mRNA水平与宫颈癌放化疗敏感性密切相关,且三者联合对放化疗敏感性具有较高的预测价值。Objective To analyze the correlation between serum squamous cell carcinoma associated antigen(SCC-Ag),human epididymal secretory protein 4(HE4),SMAD4 and radiochemotherapy sensitivity of cervical cancer.Methods A total of 74 patients with cervical cancer underwent radiochemotherapy were enrolled as the research objects.According to radiochemotherapy sensitivity,they were divided into resistance group(48 cases)and sensitivity group(26 cases).The levels of serum SCC-Ag,HE4 and SMAD4 mRNA were detected before and after treatment,relation between levels of serum SCC-Ag,HE4 and SMAD4 and ultrasound parameters were analyzed,and their predictive value for radiochemotherapy sensitivity was evaluated by receiver operating characteristic(ROC)curves.Results After treatment,mRNA levels of serum SCC-Ag and HE4 were decreased,while level of SMAD4 mRNA was increased in both groups(P<0.05).Before and after treatment,levels of SCC-Ag and HE4 in sensitivity group were lower than those in resistance group,while level of SMAD4 mRNA was higher than that in resistance group(P<0.05).Before treatment,levels of serum SCC-Ag and HE4 were positively correlated with VI(γ=0.518,0.544,P<0.05),while SMAD4 mRNA was negatively correlated with it(γ=-0.581,P<0.05).ROC curves analysis showed that predictive value of SCC-Ag combined with HE4 and SMAD4 was high for radiochemotherapy sensitivity(AUC=0.863(95%CI:0.764~0.962)).Conclusion The levels of serum SCC-Ag,HE4 and SMAD4 mRNA are closely related to radiochemotherapy sensitivity of cervical cancer,and combined detection has higher predictive value for radiochemotherapy sensitivity.
