血清β_(2)-MG、IL-17、FLC联合检测对多发性骨髓瘤早期肾损伤的诊断价值分析

摘　　要：目的研究血清β_(2)微球蛋白(β_(2)-MG)、白细胞介素-17(IL-17)、血清游离轻链(FLC)联合检测对多发性骨髓瘤早期肾损伤的诊断价值。方法选取多发性骨髓瘤患者80例,根据是否存在早期肾损伤进行分组,将肾功能正常患者纳入对照组(n=40),将肾功能异常患者纳入观察组(n=40)。两组均进行血清β_(2)-MG、IL-17、FLC的检测,观察检测结果,分析三项指标的相关性。结果观察组的血清β_(2)-MG(8.76±2.09)mg/L、IL-17水平(70.05±6.94)pg/ml、FLC-κ(1.97±0.35)mg/L、FLC-λ(2.41±0.61)mg/L、κ/λ(1.89±0.24)均高于对照组,差异具有统计学意义(P<0.05);观察组中Ⅲ期患者的β_(2)-MG、IL-17以及FLC-κ、FLC-λ、κ/λ检测值均高于Ⅱ期、Ⅰ期患者,差异具有统计学意义(P<0.05);观察组β_(2)-MG、IL-17、FLC联合检测的阳性率高于对照组,亦高于单项检测阳性率,差异具有统计学意义(P<0.05);β_(2)-MG、IL-17、FLC两两之间呈正相关(P<0.05)。结论对于多发性骨髓瘤患者早期肾损伤,联合血清β_(2)-MG、IL-17、FLC具有较好的预测和诊断价值。Objective To investigate the diagnostic value of serum β_(2) microglobulin(β_(2)-MG),interleukin-17(IL-17)and serum free light chain(FLC)combined detection in early renal injury of multiple myeloma.Methods 80 patients with multiple myeloma were selected and divided according to whether there was early kidney injury.Patients with normal kidney function were included in the control group(n=40),and patients with abnormal kidney function were included in the observation group(n=40).Serumβ_(2)-MG,IL-17 and FLC were detected in both groups,and the correlation between the three indexes was analyzed.Results Serum β_(2)-MG(8.76±2.09)mg/L,IL-17(70.05±6.94)pg/ml,FLC-κ(1.97±0.35)mg/L,FLC-λ(2.41±0.61)mg/L,κ/λ(1.89±0.24)in the observation group were higher than those in the control group.The difference was statistically significant(P<0.05).The detection values ofβ_(2)-MG,IL-17,flc-κ,FLC-λ and κ/λ in stage Ⅲ patients were higher than those in stage Ⅱ and stage Ⅰ patients in the observation group,with statistical significance(P<0.05).The positive rate of β_(2)-MG,IL-17 and FLC combined detection in the observation group was higher than that in the control group,and the positive rate of single detection was higher,the difference was statistically significant(P<0.05).There was a positive correlation between β_(2)-MG,IL-17 and FLC pairwise(P<0.05).Conclusion The combination of serum β_(2)-MG,IL-17 and FLC has a good value in predicting and diagnosing early renal injury in patients with multiple myeloma.
