低位直肠癌腹腔镜根治术中双侧入路与侧方入路疗效的比较

摘　　要：目的比较双侧入路与侧方入路低位直肠癌腹腔镜根治术的疗效。方法回顾性分析51例采用双侧入路腹腔镜根治术治疗的低位直肠癌患者临床资料,纳入双侧组;另收集51例采用侧方入路腹腔镜根治术治疗的低位直肠癌患者临床资料,纳入侧方组。比较两组患者手术相关指标、胃肠功能恢复指标以及术后并发症发生情况。结果两组患者手术时间、住院时间、淋巴结清扫数目比较,差异无统计学意义(P>0.05);双侧组患者术中出血量低于侧方组(P<0.05);双侧组患者进流质饮食时间、肠鸣音恢复时间以及排气时间均短于侧方组(P<0.05);双侧组患者术后并发症总发生率低于侧方组。结论与侧方入路比,双侧入路在低位直肠癌腹腔镜根治术中可减少患者术中出血量,有利于患者术后胃肠功能恢复,且术后并发症较少。Objective To compare therapeutic effects of laparoscopic radical resection of low rectal cancer through bilateral approach and lateral approach on patients.Methods Retrospective analysis of clinical data of 51 patients with low rectal cancer treated with bilateral laparoscopic radical surgery,included in the bilateral group.In addition,the clinical data of 51 patients with low rectal cancer treated by lateral laparoscopic radical surgery were collected and included in the lateral group.The operation related indexes,gastrointestinal function recovery indexes and postoperative complications were compared between the 2 groups.Results There was no statistical significant difference between the 2 groups in operation time,hospitalization time and number of lymph node dissection(P>0.05).The intraoperative blood volume of patients in the bilateral group was lower than that in the lateral group(P<0.05).The time of feeding fluid,recovery time of bowel sounds and exhaust time of patients in the bilateral group were shorter than those in the lateral group(P<0.05).The total incidence of postoperative complications in the bilateral group was lower than that in the lateral group(P<0.05).Conclusion Compared with the lateral approach,the bilateral approach can reduce the intraoperative bleeding in patients with low rectal cancer undergoing laparoscopic radical surgery,which is beneficial to the recovery of gastrointestinal function,and has fewer postoperative complications.
