摘  要:目的探讨固定野调强放射治疗(ff-IMRT)与螺旋断层放射治疗(HT)对宫颈癌患者近远期疗效及不良反应的影响。方法回顾性分析60例宫颈癌患者资料,根据其放疗方案将其分为IMRT组(ff-IMRT+同步化疗,n=28)和HT组(HT+同步化疗,n=32)。比较两组近远期疗效、治疗期间不良反应发生情况、放疗靶区剂量分布情况。结果两组近期疗效差异无统计学意义(P>0.05);HT组计划靶区适形度指数高于IMRT组,均匀性指数和靶区最大剂量低于IMRT组(P<0.05),靶区平均剂量两组间差异无统计学意义(P>0.05);HT组消化道不良反应和泌尿系统不良反应发生率分别为40.62%、34.38%,均低于IMRT组的75.00%、67.86%(P<0.05);两组1年内复发及死亡率差异无统计学意义(P>0.05)。结论ff-IMRT与HT均对宫颈癌患者病情有较好的控制效果,整体疗效无较大差异,但相较于ff-IMRT,HT治疗过程中辐射范围更均匀,同时在保证疗效的前提下降低辐射最大剂量,尽可能减少对周围器官组织的辐射损伤,降低放疗所致的不良反应,安全性相对更高。Objective To explore the effects of fixed-field intensity modulated radiation therapy(ff-IMRT)and helical tomotherapy(HT)on short-term and long-term efficacy and side effects in patients with cervical cancer.Methods A retrospective analysis was performed on the data of 60 patients with cervical cancer.According to different radiotherapy regimens,they were divided into IMRT group(ff-IMRT+synchronous chemotherapy,n=28)and HT group(HT+synchronous chemotherapy,n=32).The short-term and long-term efficacy,occurrence of side effects during treatment and the distribution of radiotherapy target dose were compared between the 2 groups.Results There was no significant difference in short-term efficacy between the 2 groups(P>0.05).The conformal index of planning target in HT group was higher than that in IMRT group,homogeneity index and maximum target dose were lower than those in IMRT group(P<0.05),and there was no significant difference in mean target dose between the 2 groups(P>0.05).The incidence rates of gastrointestinal and urinary system side effects in HT group were 40.62% and 34.38%,lower than those in IMRT group(75.00%,67.86%,P<0.05).There was no significant difference in recurrence or mortality between the two groups within 1 year(P>0.05).Conclusion Both ff-IMRT and HT can better control disease progression in patients with cervical cancer,without significant difference in overall efficacy.However,compared with ff-IMRT,the radiation range is more homogeneous in HT treatment,which can reduce the maximum radiation dose while ensuring efficacy,minimize radiation damage to surrounding organs and tissues,and reduce radiotherapy-induced side effects,with relatively higher safety.
