血清PS、Lp-PLA2水平与脑卒中患者失语的关系

摘　　要：目的探讨血清P选择素(PS)、脂蛋白磷脂酶A2(Lp-PLA2)水平与脑卒中患者失语的关系。方法选取2021年1月至2022年12月中南大学湘雅医学院附属株洲医院收治的110例脑卒中患者,根据是否发生失语分为失语组和言语正常组。收集两组患者基础资料,检测血清PS、Lp-PLA2水平,分析血清PS、Lp-PLA2水平与脑卒中患者失语的关系。结果110例脑卒中患者失语发生率为29.09%(32/110)。失语组患者格拉斯哥昏迷指数评分(GCS)明显低于言语正常组,左侧病变、认知障碍患者比例也高于言语正常组(P<0.05)。失语组患者血清PS、Lp-PLA2水平高于言语正常组(P<0.05)。失语患者血清PS、Lp-PLA2水平与西部失语成套测验,(WAB)评分呈显著负相关(P<0.05)。Logistic回归分析显示,入院GCS评分低、左侧病变、认知障碍、血清PS和Lp-PLA2水平升高是脑卒中患者失语的重要影响因素(P<0.05)。结论脑卒中失语患者血清PS、Lp-PLA2水平升高,其与失语风险、病情严重程度密切相关。Objective To investigate the relationship between serum P-selectin(PS),lipoprotein phospholipase A2(Lp-PLA2)levels and aphasia in patients with stroke.Methods A total of 110 patients with stroke admitted to Xiangya Hospital Zhuzhou central south university from January 2021 to December 2022 were selected and divided into aphasia group and normal speech group according to whether aphasia occurred.The basic data of two groups of patients were collected and serum PS and Lp-PLA2 levels were detected.The relationship between serum PS,Lp-PLA2 levels and aphasia in patients with stroke was analyzed.Results The incidence of aphasia in 110 patients with stroke was 29.09%(32/110).The Glasgow Coma index score(GCS)in aphasia group was significantly lower than that in normal speech group,and the proportion of patients with left side lesions and cognitive impairment was also higher than that in normal speech group(P<0.05).The levels of PS and Lp-PLA2 in aphasia group were higher than those in normal speech group(P<0.05).The levels of serum PS and Lp-PLA2 were negatively correlated with WAB score in aphasia patients(P<0.05).Logistic regression analysis showed that low admission GCS score,left side lesions,cognitive impairment,elevated serum PS and Lp-PLA2 levels were the main influencing factors of aphasia in patients with stroke(P<0.05).Conclusion The levels of serum PS and Lp-PLA2 increase in patients with aphasia after stroke,which is closely related to the risk of aphasia and the severity of stroke.
