摘　　要：目的探讨三阴型乳腺癌患者血清外泌体中信使RNA(mRNA)突触后密度蛋白95(PSD-95)的表达及其与患者病理特征的关系。方法收集三阴型乳腺癌与乳腺良性肿瘤(乳腺纤维腺瘤、瘤样增生)患者血清外泌体各4例,通过高通量芯片和基因芯片相结合的方法,筛选出在三阴型乳腺癌与乳腺良性肿瘤患者血清外泌体中有显著差异的mRNAs。通过富集分析了解各mRNA的功能富集情况,并从中发现具有差异表达的mRNA(PSD-95,又称DLG4)富集在NF-κB信号通路中,该通路与肿瘤的侵袭转移相关。采用免疫组织化学染色法对30例乳腺良性肿瘤组织及36例三阴型乳腺癌患者的组织病理切片进行分析,并验证PSD-95在三阴型乳腺癌中的表达水平及其与临床病理特征的相关性。结果PSD-95在三阴型乳腺癌组织中的表达水平显著低于乳腺良性肿瘤组织(P<0.01)。相关性分析结果显示,PSD-95的差异表达与患者淋巴结转移情况明显相关(r=-0.514,P<0.05),而与患者的年龄、绝经状态、Ki67表达水平、肿瘤大小(T)及组织学分级等无关。结论PSD-95在三阴型乳腺癌中的表达显著下调,其与患者淋巴结转移呈负相关,可能与乳腺癌的侵袭转移有关。Objective To investigate the expression of messenger RNA(mRNA)postsynaptic density protein 95(PSD-95)in serum exocrine of the patients with triple negative breast cancer and its relationship with pathological features.Methods Theserum exocrine bodies were collected from 4 patients with triple negative breast cancer and 4 patients with benign breast tumor(fibroadenoma and tumor-like hyperplasia).The mRNAs with significant difference between triple negative breast cancer and benign breast tumor was screened by high-throughput microarray and gene chip.The functional enrichment of each mRNA was understood by enrichment analysis,and it was found that the differentially expressed mRNA(PSD-95,also known as DLG4)was enriched in the NF-κB signal pathway,which was related to tumor invasion and metastasis.The histopathological sections of 30 cases of benign breast tumor and 36 cases of triple negative breast cancer were analyzed by immunohistochemical staining,and the expression level of PSD-95 in triple negative breast cancer and its correlation with clinicopathological features were verified.Results The expression level of PSD-95 in the tissues of the patients with triple negative breast cancer was significantly lower than that in patients with benign breast tumors.The results of correlation analysis showed that the differential expression of PSD-95 was significantly correlated with lymph node metastasis(P<0.05).There was no correlation with age,menopausal status,Ki67 expression level,tumor size(T)and histological grade.Conclusion The expression of DLG4 in triple negative breast cancer is significantly down-regulated,which is negatively correlated with lymph node metastasis,and it may be related to the invasion and metastasis of breast cancer.
