摘 要:目的:探究视频脑电图监测(Video Electroencephalography,VEEG)与磁共振成像(Magnetic Resonance Imaging,MRI)应用于癫痫患儿定位诊断中的价值。方法:选取2021年12月-2022年12月于我院进行病灶切除术的小儿癫痫患者100例进行回顾性分析研究。所有患儿均行VEEG与MRI检测。以病灶切除术的术中结果(病理检测)作为金标准,将两种检查方式的异常情况检出结果和致痫灶定位结果与术中结果进行对比。计算其异常检出率和一致率,并比较其诊断效能。结果:VEEG检出79例异常者,其中局灶性放电75例。MRI检出53例异常者,其中局灶性异常者50例。VEEG的异常检出率(79.00%)高于磁共振成像(53.00%)(P<0.05)。将VEEG与MRI发现的致痫灶与手术中结果相对比,VEEG和联合诊断的总一致率均明显高于MRI(P<0.05)。结论:VEEG应用于癫痫患儿定位诊断中的诊断效能良好,可明显提升对癫痫患儿脑部异常情况的检出率和病灶定位的准确率。Objective:To explore the value of Video Electroencephalography(VEEG)and Magnetic Resonance Imaging(MRI)in the localization diagnosis of children with epilepsy.Methods:100 cases of pediatric epilepsy patients who underwent focal resection in our hospital from December 2021 to December 2022 were retrospectively analyzed.VEEG and MRI were performed in all patients.Using the intraoperative results of lesion resection(pathological examination)as the gold standard,the results of abnormal detection and epileptogenic focus location of the two examination methods were compared with the intraoperative results.The anomaly detection rate and consistency rate were calculated,and the diagnostic efficiency was compared.Results:79 abnormal patients were detected by VEEG,of which 75 were focal discharges.There were 53 abnormalities detected by MRI,of which 50 were focal abnormalities.The abnormal detection rate of VEEG(79.00%)was higher than that of MRI(53.00%)(P<0.05).The overall concordance rate of VEEG and combined diagnosis was significantly higher than that of MRI(P<0.05)when compared with the epileptogenic focus found by VEEG and MRI.Conclusion:VEEG has good diagnostic efficacy in the localization diagnosis of children with epilepsy,and can significantly improve the detection rate of brain abnormalities and the accuracy of focal localization in children with epilepsy.
