经颅磁刺激辅助针刺八髎穴、曲泉穴治疗中风后尿潴留效果观察

摘　　要：目的:探析经颅磁刺激辅助针刺八髎穴、曲泉穴治疗中风后尿潴留的应用效果。方法:纳入2019年6月至2022年8月医院收治的中风后并发尿潴留患者,随机分为对照组和观察组,各43例。对照组在常规治疗基础上采用针刺八髎穴和曲泉穴治疗,观察组在对照组基础上采用经颅磁刺激辅助治疗,两组均连续治疗2 w。比较两组治疗效果,以及治疗前、治疗2 w后两组患者排尿情况(排尿次数、日均单次尿量)、尿动力学指标(膀胱残余尿量、最大尿流速、膀胱最大容量)。结果:观察组总有效率高于对照组,差异有统计学意义(P<0.05);与治疗前相比,两组治疗2 w后排尿次数、日均单次尿量均明显增加(P<0.05),其中观察组更为显著(P<0.05);与治疗前相比,两组治疗2 w后最大尿流率、膀胱最大容量高均明显增加,膀胱残余尿量明显降低(P<0.05),其中观察组更为显著(P<0.05)。结论:经颅磁刺激辅助针刺八髎穴、曲泉穴治疗中风后尿潴留疗效显著,可增加患者排尿次数、日均单次尿量、最大尿流率、膀胱最大容量,减少膀胱残余尿量,提高治疗效果。Objective:To explore the effect of transcranial magnetic stimulation assisted acupuncture at Baliao and Ququan on urinary retention after stroke.Methods:The patients with urinary retention after stroke admitted to the hospital from June 2021 to August 2022 were included.They were randomly divided into the control group and the observation group with 43 cases each.The control group was treated with acupuncture at Baliao and Ququan points on the basis of routine treatment,while the observation group received transcranial magnetic stimulation as an auxiliary treatment on the basis of the control group.Both groups were treated continuously for 2 weeks.The treatment effects of the two groups,as well as the urination status(urination frequency,daily average single urine volume)and urodynamic indicators(residual urine volume,maximum urine flow rate,and maximum bladder volume)of the two groups of patients before and after 2 weeks of treatment were compared.Results:The total effective rate of the observation group was higher than that of the control group,with a statistical significant difference(P<0.05).Compared with before treatment,both groups showed a significant increase in urination frequency and daily single urine output after 2 weeks of treatment(P<0.05).Among them,the observation group was more significant(P<0.05).Compared with before treatment,after 2 weeks of treatment,the maximum urine flow rate and maximum bladder volume in both groups were significantly increased,and the residual urine volume in the bladder was significantly reduced(P<0.05).Among them,the observation group was more significant(P<0.05).Conclusion:Transcranial magnetic stimulation assisted acupuncture at Baliao and Ququan points has a significant effect on Urinary retention after stroke,which can increase the number of urinations,daily average single urine volume,maximum urinary flow rate,and maximum capacity of the bladder,reduce residual urine volume of the bladder,and improve the therapeutic effect.
