摘 要:目的:分析氟-氟代脱氧葡萄糖(^(18)F-fluorodeoxyglucose,^(18)F-FDG)正电子发射型计算机断层显像(Positron emission tomography/computed tomography,PET/CT)影像组学特征多参数模型在非霍奇金淋巴瘤患者预后评估的诊断价值。方法:选取本院于2021年5月至2022年8月期间收治的80例临床诊断为非霍奇金淋巴瘤的患者作为研究对象。患者均接受化疗,治疗后采用CT,^(18)F-FDG PET/CT进行扫描分析。以病理学诊断结果作为金标准,分析^(18)F-FDG PET/CT的诊断价值。分析各代谢参数对患者预后的诊断价值。结果:CT扫描,阳性诊断率为67.50%;^(18)F-FDG PET/CT扫描,阳性诊断率为67.50%,两种方法不具有显著性差异(P>0.05)。^(18)F-FDG PET/CT方法的特异性、灵敏度、准确度和阳性预测值均明显高于常规CT扫描方法,均具有显著性差异(P<0.05);在PET/CT参数中,总糖酵解量和肿瘤代谢体积可作为无进展生存期(Progression free survival,PFS)的影响条件(P<0.05);美国东部肿瘤协作组评分(P<0.05)可作为非霍奇金淋巴瘤患者PFS的预后影响条件。结论:采用化疗方案治疗非霍奇金淋巴瘤患者,^(18)F-FDG PTE/CT显像技术用于评估这类淋巴瘤的疗效具有较高的准确率。Objective:Analysis of fluorine-fluoro deoxidization glucose(f-18 fluorodeoxyglucose,18 f-FDG)type positron emission computed tomography(ct)imaging(positron emission tomography/computed tomography),The diagnostic value of multi-parameter models of PET/CT imaging features in prognostic assessment of patients with non-Hodgkin's lymphoma.Methods:A total of 80 patients with non-Hodgkin lymphoma who were clinically diagnosed in our hospital from May 2021 to August 2022 were selected as the study subjects.All patients received chemotherapy and were analyzed by CT,^(18)F-FDG PET/CT scan after treatment.The diagnostic value of ^(18)F-FDG PET/CT was analyzed with pathological diagnosis as the gold standard.To analyze the diagnostic value of each metabolic parameter to the prognosis of patients.Results:CT scan,the positive diagnosis rate was 67.50%.The positive diagnosis rate of ^(18)F-FDG PET/CT scan was 67.50%,and there was no significant difference between the two methods(P>0.05).The specificity,sensitivity,accuracy and positive predictive value of ^(18)F-FDG PET/CT method were significantly higher than those of conventional CT scanning method(P<0.05).Among the PET/CT parameters,total glycolysis volume and tumor metabolic volume could be used as the influencing conditions for progression free survival(PFS)(P<0.05).The Eastern United States Tumor Collaborative Group score(P<0.05)can be used as a prognostic condition for PFS in patients with non-Hodgkin lymphoma.Conclusion:^(18)F-FDG PTE/CT imaging is highly accurate in evaluating the efficacy of chemotherapy in patients with non-Hodgkin lymphoma.
