T淋巴细胞及EGFR与肺腺癌临床特征及预后的关系分析

摘　　要：目的:探讨T淋巴细胞亚群及表皮生长因子受体(Epidermal growth factor receptor,EGFR)与肺腺癌临床特征及预后的关系。方法:选取本院2019年10月至2022年10月收治的72例肺腺癌患者作为研究对象。比较两组患者肺腺癌病理组织与癌旁组织的CD4^(+)、CD8^(+)和EGFR表达水平,分析其与临床特征的关系。采用受试者工作特征曲线(Receiver Operating Characteristic,ROC)评估CD4^(+)、CD8^(+)和EGFR表达水平对肺腺癌的诊断价值。采用Kaplan-Meier法绘制生存曲线,探讨T细胞亚群和EGFR表达水平与肺腺癌患者预后的关系。结果:肺腺癌病理组织的CD8^(+)含量及EGFR表达水平均明显高于癌旁组织,而CD4^(+)含量明显低于癌旁组织(P<0.05)。CD4^(+)、CD8^(+)含量及EGFR表达水平与淋巴结是否转移和不同TNM分期及分化程度有关(P<0.05)。ROC曲线结果显示:CD4^(+)、CD8^(+)和EGFR的AUC值分别为0.547、0.723、0.757,对肺腺癌患者具有一定的诊断价值。高CD4^(+)含量的生存时间比低CD4^(+)含量长,高CD8^(+)含量的生存时间比低CD8^(+)含量短,EGFR阳性的生存时间比阴性短。结论:肺腺癌患者的CD8^(+)和EGFR在机体内呈现高表达水平,而CD4^(+)呈现低表达水平,均与患者临床特征和预后密切相关。Objective:To investigate the relationship of T lymphocyte subsets and Epidermal growth factor receptor(EGFR)with clinical features and prognosis of lung adenocarcinoma.Methods:72 patients with lung adenocarcinoma admitted to our hospital from October 2019 to October 2022 were selected as the study objects.The expression levels of CD4^(+),CD8^(+)and EGFR in pathological tissues and adjacent tissues of lung adenocarcinoma were compared between the two groups,and their relationship with clinical features was analyzed.The value of CD4^(+),CD8^(+)and EGFR expression levels in the diagnosis of lung adenocarcinoma was evaluated by Receiver Operating Characteristic(ROC).Kaplan-Meier method was used to plot the survival curve to explore the relationship between T cell subsets and EGFR expression level and prognosis of lung adenocarcinoma patients.Results:The level of CD8^(+)and EGFR expression in pathological tissue of lung adenocarcinoma was significantly higher than that in paracancer tissue,while the level of CD4^(+)was significantly lower than that in paracancer tissue(P<0.05).The levels of CD4^(+),CD8^(+)and EGFR expression were correlated with lymph node metastasis,TNM stage and differentiation degree(P<0.05).ROC curve results showed that the AUC values of CD4^(+),CD8^(+)and EGFR were 0.547,0.723 and 0.757,respectively,which had certain diagnostic value for lung adenocarcinoma patients.The survival time of high CD4^(+)was longer than that of low CD4^(+),the survival time of high CD8^(+)was shorter than that of low CD8^(+),and the survival time of EGFR-positive was shorter than that of negative.Conclusion:The expression of CD8^(+)and EGFR is high in lung adenocarcinoma patients,while the expression of CD4^(+)is low,which is closely related to the clinical characteristics and prognosis of patients.
