血清乳酸脱氢酶对神经母细胞瘤患者的临床意义

摘  要:目的:探讨治疗前血清乳酸脱氢酶水平与神经母细胞瘤患者的预后关系。方法:选取2013年11月—2017年12月郑州大学附属儿童医院收治的47例确诊为神经母细胞瘤患者的临床资料,应用卡方检验进行乳酸脱氢酶水平与神经母细胞瘤临床病理特点进行比较,应用Kaplan-Meier方法进行生存分析,应用COX回归分析进行多因素分析。结果:生存分析结果显示,不同乳酸脱氢酶患者3年总生存率低水平组为85.0%,中水平组为50.0%,高水平组为30.8%。3年无事件生存率低水平组为80.0%,中水平组为35.7%,高水平组为23.1%。多因素分析结果显示,治疗前血清乳酸脱氢酶水平不能作为神经母细胞瘤患者的独立预后指标,差异无统计学意义(P>0.05)。结论:治疗前血清乳酸脱氢酶水平可作为评估神经母细胞瘤患者的预后指标之一。Objective:To investigate the relationship between pretreated serum lactate dehydrogenase and prognosis of neuroblastoma.Methods:Patients diagnosed as neuroblastoma from November 2013 to December 2017 were collected.Chi-square test was used to compare lactate dehydrogenase level with clinicopathological characteristics of neuroblastoma.Kaplan-Meier method was used for survival analysis,and COX regression analysis was used for multivariate analysis.Results Survival analysis showed that the 3-year overall survival rate of patients with different lactate dehydrogenase was 85.0%in the low level group,50.0%in the medium level group,and 30.8%in the high level group.3-year event-free survival rate was 80.0%in the low level group,35.7%in the medium level group,and 23.1%in the high level group.Multivariate analysis showed that pretreated lactate dehydrogenase level could not be used as an independent prognostic factor for patients with neuroblastoma,with no statistically significant difference(P>0.05).Conclusion The pretreated serum lactate dehydrogenase level can be used as one of the prognostic factor for neuroblastoma.
