糖皮质激素在老年带状疱疹性角膜炎中的疗效

摘　　要：目的对糖皮质激素在老年带状疱疹性角膜炎中的临床运用效果情况进行观察与探析。方法抽取2021年4月至2022年4月所接诊的86例老年带状疱疹性角膜炎患者作为观察对象,按随机数字表法分成两组,对照组采取常规药物(阿昔洛韦、甲钴胺片、更昔洛韦滴眼液、更昔洛韦眼用凝胶等)治疗,在此基础之上,观察组采取糖皮质激素地塞米松治疗。对以上两组患者的总有效率、症状改善时间、视力水平等指标进行比较。结果观察组治疗后的总有效率比对照组高(χ^(2)=4.074,P<0.05);观察组治疗后畏光消退时间、疼痛消退时间、角膜伤愈合时间、眼睛不适消退时间、虹膜炎消退时间以及荧光染色转阴时间等均低于对照组(P<0.05);观察组治疗前的视力水平与对照组相比较,差异无统计学意义(P>0.05);相较于治疗前,观察组与对照组治疗后的视力水平均有所提高,且治疗后观察组的视力水平高于对照组(P<0.05);观察组不良反应发生率比对照组低(χ^(2)=4.441,P<0.05)。结论对老年带状疱疹性角膜炎患者予以糖皮质激素治疗的效果甚佳,能有效缓解患者的临床症状,提高患者的视力水平,减少不良反应的发生。Objective To observe and analyze the clinical application effect of glucocorticoid in senile herpes zoster keratitis.Methods Extracted from April 2021 to April 2022 received 86 cases of elderly patients with herpes zoster keratitis as the observation subject,according to the random number table method into two groups,the control group take conventional drugs(acyclovir,methylcobalamin tablets,ganciclovir eye drops,ganciclovir,eye gel,etc.)treatment,on this basis,the observation group take glucocorticoid dexamethasone treatment.The indexes of total response rate,time to symptom improvement,and visual acuity level were compared between the above two groups.Results The total effective rate of the observation group after treatment was higher than that of the reference group(χ^(2)=4.074,P<0.05).After treatment,the photophobia fading time,pain fading time,corneal wound healing time,eye discomfort fading time,iriditis fading time and fluorescent staining negative turning time in the observation group were lower than those in the reference group(P<0.05).Compared with the reference group,the visual acuity of the observation group before treatment showed no significant difference(P>0.05).Compared to before treatment,the visual acuity level of observation group and control group was improved,and the visual acuity level of observation group was higher than that of control group after treatment(P<0.05).The incidence of adverse reactions in the observation group was lower than that in the reference group(χ^(2)=4.441,P<0.05).Conclusions Glucocorticoid therapy is effective for elderly patients with herpes zoster keratitis,which can effectively alleviate the clinical symptoms of patients,improve their visual acuity and reduce the occurrence of adverse reactions.
