老年复发性三叉神经痛患者心理痛苦状况及其影响因素

摘　　要：目的统计老年复发性三叉神经痛(RTN)患者心理痛苦状况,并分析其影响因素。方法选取于2022年6月至2023年6月至信阳市中心医院疼痛科就诊的老年RTN患者进行单中心横断面调查,通过心理痛苦温度计(DT)、家庭支持自评量表(PSS-Fa)、匹兹堡睡眠质量指数(PSQI)、自制资料调查表采集老年RTN患者心理痛苦状况、家庭支持程度、睡眠质量及人口学特征;应用单因素和多因素分析探讨影响老年RTN患者心理痛苦状况的因素。结果102例老年RTN患者中,DT评分0~8分,平均(4.28±1.74)分,其中DT<4分44例,占比43.14%;DT≥4分58例,占比56.86%。DT≥4分组年龄、PSQI评分大于DT<4分组,双支或多支疼痛支数占比高于DT<4分组,PSS-Fa评分低于DT<4分组(P<0.05)。经logistic回归分析结果显示年龄大、疼痛支数多、睡眠质量差是老年RTN患者心理痛苦的危险因素(OR>1,P<0.05);家庭支持程度高是老年RTN患者心理痛苦的保护因素(OR<1,P<0.05)。结论老年RTN患者普遍存在心理痛苦,年龄大、疼痛支数多、睡眠质量差、家庭支持程度低是老年RTN患者心理痛苦产生的影响因素。Objective To investigate the psychological pain status of elderly patients with recurrent trigeminal neuralgia(RTN)and analyze its influencing factors.Methods A single center cross-sectional survey was conducted on elderly RTN patients who visited the Department of Pain Management of Xinyang Central Hospital from June 2022 to June 2023.The psychological distress status,level of family support,sleep quality and demographic characteristics of elderly RTN patients were collected using the distress thermometer(DT),family support scale(PSS-Fa),Pittsburgh sleep quality index(PSQI)and self-made data questionnaires.Univariate and multivariate analysis were used to explore the factors affecting the psychological distress of elderly patients with RTN.Results According to statistics,among 102 elderly RTN patients,the DT score was 0-8 points,with an average of(4.28±1.74)points.Among them,44 cases had DT scores less than 4 points,accounting for 43.14%,and 58 cases with DT≥4 points,accounting for 56.86%.The age and PSQI score of the DT≥4 group were higher than those of the DT<4 group,and the proportion of double or multiple pain branches was higher than that of the DT<4 group.The PSS-Fa score was lower than that of the DT<4 group(P<0.05).The results of logistic regression analysis showed that older age,more pain and poor sleep quality were risk factors for psychological pain in elderly RTN patients(OR>1,P<0.05).A high level of family support was a protective factor for psychological pain in elderly RTN patients(OR<1,P<0.05).Conclusion Elderly RTN patients generally experience psychological pain,and older age,more pain,poor sleep quality,and low level of family support are the influencing factors for the psychological pain of elderly RTN patients.
