论暑热泄泻

摘　　要：泄泻常见于夏秋季节,大人和儿童均可出现,夏月当令,暑邪和湿邪确为主要病邪。湿邪所致泄泻常有饮食不节或者久居湿地的病因,表现为胸脘满闷、肢体倦怠、不思饮食、泄泻、舌苔厚腻等湿邪内停的征象,此类泄泻的记载良多。暑邪侵袭导致的泄泻,历代医家少有论述。暑热泄泻的发生先有感受暑邪的病因,患者或者在暑热天气下作业,或者长时间待在闷热的房间内,暑邪首先犯表,侵袭太阳表经,易袭于阳明,二阳相和,从而出现“发热,汗出,口渴,脉洪大”的阳明经证,导致阳明里气逆乱,从而出现胃肠升降失序,水谷不分,下祛大肠,最后发为泄泻。暑热泄泻的治疗常囿于“无湿不成泻”“暑兼湿热”的治疗思路,常与湿热泄泻相混淆。发生暑热泄泻时,如果用利湿诸法治疗,不仅不能治愈疾病,还会加重病情,导致津液耗伤,甚至昏狂等。文章基于六经理论,从暑邪的性质、暑邪致泻的机制、暑热泄泻与湿热泄泻的鉴别、暑热泄泻的治法、医案举例、结语这六方面分别阐述暑热泄泻这一疾病,以期为此类泄泻的治疗提供新的思路和方向。Diarrhea is common in summer and autumn seasons,both adults and children can be infected with this disease,and summer-heat pathogenic factor and damp pathogenic factor are the main pathogenic factors.The main causes of diarrhea caused by dampness are improper diet or long-term residence in wetlands,which is manifested as chest cavity full tightness,body fatigue,no appetite,diarrhea,tongue coating thick and greasy signs and other stagnated dampness symptoms.There are many records about this kind of diarrhea,while the doctors in the past dynasties rarely discussed the diarrhea due to the invasion of heat pathogenic factor in summer.The cause of summer-heat diarrhea is the attack of summer-heat pathogenic factor.For example,when patients work in hot weather or stay in the stuffy room for a long time,the heat pathogenic factor first attacks the Taiyang(太阳)surface meridian,and then attacks the Yangming(阳明)meridian,and the two Yang(阳)meridians are attacked at the same time,resulting in the Yangming meridian symptoms of fever,sweating,thirst,and heavy pulse,which causes Qi(气)disorder in Yangming meridian.Due to gastrointestinal dysfunction caused by the above reasons,water and grains cannot be metabolized properly,and finally diarrhea occurs.The treatment of summer-heat diarrhea is often confined to the treatment ideas of“Diarrhea is caused by dampness”and“The heat and dampness invade human body simultaneously”,which is often confused with dampness-heat diarrhea.Various methods of dispelling dampness can not cure summer-heat diarrhea,but often aggravate the condition,leading to body fluid depletion,and even dizziness.Based on the six meridians theory,this paper expounds the disease from the six aspects of the nature of summer-heat diarrhea,the action mechanism of summer-heat pathogenic factor causing diarrhea,the identification of summer-heat diarrhea and dampness-heat diarrhea,the treatment method of summer-heat diarrhea,medical examples,and the conclusion,in order to provide new ideas and direction
