摘 要:目的分析教学医院临床医师工作角色转化过程中角色压力的具体表现及成因,提出相应对策。方法于2023年7—9月通过目的抽样法选取武汉市3所三级甲等教学医院不同科室的10名临床医师进行面对面半结构化访谈,采用归纳式内容分析法提炼和总结其角色压力的具体表现及成因。结果临床医师在工作角色转化中主要面临角色冲突、角色不一致、角色能力不足、角色过度负荷和角色模糊不同形式的角色压力,其成因主要包括组织层面的组织管理体系与制度不完善,资源配置不充足,沟通协调存在障碍和教学、科研发展的挑战,以及个体层面角色认知不清晰、不恰当,自身能力和角色要求不匹配等。结论建议从医院和高校层面完善组织管理体系,促进资源合理配置,加强院校间沟通协调,优化教师队伍建设与管理模式,丰富培训和考核形式等。临床医师层面需要采取强化职业认同、及时进行角色重构、终身持续学习、保持积极心态、寻求团队协作等方式,有效应对角色压力。Objective Analyze the specific manifestations and causes of role stress in the process of role transformation of clinical physicians in teaching hospitals and propose corresponding strategies.Methods From July to September 2023,a purposive sampling method was employed to select 10 clinical physicians from different departments of three tertiary A-level teaching hospitals in Wuhan for face-to-face semi-structured interviews.Inductive content analysis was used to extract and summarize the specific manifestations and causes of their role stress.Results Clinical physicians,during the transformation of their work roles,primarily face various forms of role stress,including role conflicts,role inconsistencies,inadequate role capabilities,role overload,and role ambiguity.The causes are mainly related to organizational factors such as imperfect organizational management systems and inadequate resource allocation,communication and coordination obstacles,challenges in teaching and research development,as well as individual factors such as unclear and inappropriate role cognition and mismatch between personal abilities and role requirements.Conclusion It is recommended to improve organizational management systems at the hospital and school levels,promote rational resource allocation,strengthen communication and coordination between institutions,optimize teacher team construction and management models,and diversify training and assessment methods.At the individual level,clinical physicians should enhance professional identity,engage in timely role reconstruction,commit to lifelong continuous learning,maintain a positive mindset,and seek collaborative teamwork to effectively address role stress.
