明目熏眼方雾化熏蒸治疗肝肾阴虚型干眼症临床研究

摘　　要：目的观察明目熏眼方雾化熏蒸治疗肝肾阴虚型干眼症的临床疗效。方法选取2021年6月至2022年6月收治的62例肝肾阴虚型干眼症患者为研究对象,按照随机数字表法分为2组,在均予玻璃酸钠滴眼液联合睑板腺按摩治疗的基础上,对照组31例(62眼)予蒸馏水雾化熏蒸治疗,治疗组31例(62眼)予明目熏眼方雾化熏蒸治疗。2组均治疗4周后统计疗效,比较2组治疗前后眼表疾病指数(OSDI)评分、泪液分泌试验、泪膜破裂时间(BUT)、角膜荧光素钠染色(FL)评分、明视持久度及美国国立眼科研究所视觉相关生命质量量表(NEI-vFQ-25)评分变化情况,并统计2组治疗期间睑板腺功能异常检出率。结果与本组治疗前比较,2组治疗后OSDI评分均降低(P<0.05),且治疗组治疗后OSDI评分低于对照组(P<0.05)。与本组治疗前比较,2组治疗后泪液分泌试验长度均增加(P<0.05),且治疗组治疗后泪液分泌试验长度大于对照组(P<0.05)。与本组治疗前比较,2组治疗后BUT均变长(P<0.05),且治疗组治疗后BUT长于对照组(P<0.05)。与本组治疗前比较,2组治疗后FL评分均降低(P<0.05),且治疗组治疗后FL评分低于对照组(P<0.05)。与本组治疗前比较,2组治疗后明视持久度均升高(P<0.05),且治疗组治疗后明视持久度高于对照组(P<0.05)。与本组治疗前比较,2组治疗后NEI-VFQ-25评分均升高(P<0.05),且治疗组治疗后NEI-VFQ-25评分高于对照组(P<0.05)。治疗组睑板腺功能异常检出率20.97%(13/62),对照组睑板腺功能异常检出率38.71%(24/62),治疗组睑板腺功能异常检出率低于对照组(P<0.05)。结论明目熏眼方雾化熏蒸治疗干眼症疗效确切,可有效改善患者眼部症状,提高泪膜稳定性,保护角膜上皮细胞完整,增加明视持久度,改善生活质量,降低睑板腺功能异常风险。Objective To observe the efficacy of fumigation therapy of Mingmu Xunyan Recipe on dry eye syndrome of liver-kidney yin deficiency type.Methods Sixty-two patients with dry eye syndrome(124 eyes)treated in our hospital from June 2021 to June 2022 were enrolled,they were randomized 1∶1 to the control group(n=31,62 eyes)or research group(n=31,62 eyes).All patients were given massage at meibomian gland,and those in the control group and the research group were additionally given fumigation therapy of distilled water and self-made Mingmu Xunyan Recipe,respectively.The curative effect was statistically analyzed after 4 weeks of treatment.The ocular surface disease index(OSDI)score,tear secretion test(TST),tear break-up time(BUT),corneal fluorescein staining(FL)score,and duration of photopic vision(DPV)were included as comparators before and after treatment between groups.The detection rate of meibomian gland dysfunction during treatment was counted.Results After treatment,the patients in the both groups presented significantly decreased OSDI scores(P<0.05),significantly increased TST(P<0.05),significantly longer BUT(P<0.05),significantly reduced FL score(P<0.05),significantly increased DPV(P<0.05),and significantly increased National Eye Institute Visual Functioning Questionnaire-25(NEI VFQ-25)score(P<0.05)than those before treatment(P<0.05),and the improvements were more pronounced in the treatment group relative to the control group(all P<0.05).The detection rate of meibomian gland dysfunction in the treatment group was significantly lower than that in the control group(20.97%[13/62]vs 38.71%[24/62],P<0.05).Conclusion The fumigation therapy of self-made Mingmu Xunyan Recipe is effective in treating dry eye syndrome.It can effectively improve patients'eye symptoms,improve tear film stability,protect the integrity of corneal epithelial cells,increase the duration of photopic vision,improve quality of life,and reduce risk for meibomian gland dysfunction.
