摘　　要：目的探讨溶酶体相关4次跨膜蛋白B-35(LAPTM4B-35)、Dickkopf相关蛋白1(DKK1)在肝肿瘤血清中的表达以及联合低剂量超声造影在肝肿瘤良恶性诊断中的应用价值。方法收集2021年1月至2022年8月青岛市中心医院急诊创伤外科进行治疗的160例肝肿瘤患者临床资料。根据病理学结果,将患者分为良性组(n=74)和恶性组(n=86)。比较两组血清LAPTM4B-35、DKK1水平及低剂量超声造影指标;多因素Logistic回归分析肝恶性肿瘤的影响因素;ROC曲线分析血清LAPTM4B-35、DKK1联合低剂量超声造影对肝恶性肿瘤的诊断效能。结果恶性组患者血清LAPTM4B-35、DKK1表达水平均高于良性组(P<0.05)。恶性组达峰时间低于良性组(P<0.05),峰值强度显著高于良性组(P<0.05)。多因素Logistic回归分析,结果显LAPTM4B-35、DKK1、达峰时间、峰值强度是发生肝恶性肿瘤的影响因素(P<0.05)。ROC曲线显示,血清LAPTM4B-35、DKK1、低剂量超声造影联合检测诊断肝恶性肿瘤的AUC为0.960,敏感性为89.53%,特异性为91.89%,优于其各自单独预测(Z两者联合-LAPTM4B-35=5.116、Z两者联合-DKK1=4.986、Z两者联合-达峰时间=4.657、Z两者联合-峰值强度=4.214,P<0.05)。结论肝肿瘤恶性患者血清LAPTM4B-35、DKK1表达水平显著升高,联合低剂量超声造影在肝恶性肿瘤的诊断中具有较高的应用价值。Objective To investigate the expression of lysosome associated transmembrane protein 4B-35(LAPTM4B-35)and Dickkopf related protein 1(DKK1)in serum of liver tumors and the applicationvalue of combined low-dose contrast-enhanced ultrasound in the diagnosis of benign and malignant liver tumors.Methods From January 2021 to August 2022,it was collected the clinical data of 160 patients with liver tumor who were treated in the emergency trauma surgery Department of Qingdao Central Hospital.Patients were divided into benign group(n=74)and malignant group(n=86)based on pathological findings.The levels of serumLAPTM4B-35,DKK1 and low-dose contrast-enhanced ultrasound were compared between the two groups.The influencing factors of hepatic malignant tumor were analyzed by multivariate Logistic regression.ROC curve was used to analyze the diagnostic efficacy of serum LAPTM4B-35 and DKK1 combined with low-dose contrast-enhanced ultrasound in hepatic malignancies.Results The levels of LAPTM4B-35 and DKK1 in the malignant group were higher than those in the benign group(P<0.05).The peak time of the malignant group was lower than that of the benign group(P<0.05),and the peak intensity was significantly higher than that of the benign group(P<0.05).Multivariate Logistic regression analysis showed that LAPTM4B-35,DKK1,peak time and peak intensity were the factors affecting the occurrence of liver malignant tumor(P<0.05).ROC curve showed that the AUC of serum LAPTM4B-35,DKK1 and low-dose contrast-enhanced ultrasound combined detection for the diagnosis of liver malignant tumors was 0.960,the sensitivity was 89.53%,and the specificity was 91.89%.The results were better than those predicted separately(Z combined with LAPTM4B-35=5.116,Z combined with DKK1=4.986,Z combined with Z-peak time=4.657,Z combined with Z-peak intensity=4.214,P<0.05).Conclusion The expression levels of LAPTM4B-35 and DKK1 in serum of patients with malignant liver tumors are significantly increased.Combined with low-dose contrast-enhanced ultrasound has high appl
