摘 要:目的 探究急性胰腺炎(AP)患者血清中三叶因子2(TFF2)、微小RNA-186-5p(miR-186-5p)表达水平与疾病严重程度的关系。方法 选取2021年3月—2022年3月沧州市中心医院收治的AP患者110例作为研究对象,根据病情严重程度分为轻症组60例和重症组50例,同时选取55名在本院体检健康人员作为对照组,比较三组血清TFF2、miR-186-5p水平。采用Pearson相关性分析血清TFF2、miR-186-5p水平与hs-CRP、IL-18、TNF-α水平和APACHEⅡ评分的相关性。受试者工作特征曲线(ROC)分析血清TFF2、miR-186-5p水平对重症SAP的诊断价值。采用Logistic回归分析影响重症AP的因素。结果 重症组TFF2、hs-CRP、IL-18、TNF-α水平和APACHEⅡ评分均高于轻症组,miR-186-5p水平低于轻症组(P<0.05)。相关性分析显示,miR-186-5p与TFF2有靶向结合位点且miR-186-5p与TFF2表达呈负相关(r=-0.479,P<0.05),AP患者血清TFF2与hs-CRP、TNF-α、IL-18、APACHEⅡ评分呈正相关(r=0.496、0.541、0.474、0.418,P<0.05);miR-186-5p与hs-CRP、TNF-α、IL-18、APACHEⅡ评分呈负相关(r=-0.421、-0.561、-0.443、-0.425,P<0.05)。ROC分析显示,TFF2、miR-186-5p辅助评估是否发生重症AP的曲线下面积(AUC)分别是0.800(95%CI:0.717~0.882)、0.824(95%CI:0.747~0.900),二者联合检测的灵敏度为80.00%,特异度为83.30%,AUC为0.888(95%CI:0.826~0.950)。多因素Logistic回归分析显示,TFF2是影响重症AP的危险因素,miR-186-5p是保护因素(P<0.05)。结论 AP患者血清TFF2水平升高,miR-186-5p表达水平降低,并且二者与AP患者病情的严重程度密切相关,可作为评估AP患者病情的分子标志物。Objective To explore the relationship between the expression levels of trefoil factor family 2(TFF2)and microRNA-186-5p(miR-186-5p)in the serum of patients with acute pancreatitis(AP)and the severity of the disease.Methods A total of 110 patients with AP admitted to our hospital from March 2021 to March 2022 were regarded as the study subjects.According to the severity of the condition,they were grouped into a mild illness group of 60 cases and a severe illness group of 50 cases,meantime,55 healthy individuals undergoing physical examination in our hospital were selected as the control group,and the serum levels of TFF2 and miR-186-5p were compared among the three groups.Pearson's method was applied to analyze the correlation of serum TFF2,miR-186-5p levels with hs CRP,IL-18,TNF-α levels,and APACHE Ⅱ scores.ROC was applied to analyze the diagnostic value of serum TFF2 and miR-186-5p levels for severe SAP.Logistic regression was applied to analyze the factors affecting severe AP.Results The levels of TFF2,hs-CRP,IL-18,TNF-α,and APACHE Ⅱ in the severe group were higher than those in the mild group,and the level of miR-186-5p was lower than that in the mild group(P<0.05).Correlation analysis showed that miR-186-5p had a targeted binding site with TFF2,and there was a negative correlation between miR 186-5p and TFF2 expression(r=-0.479,P<0.05),there was a positive correlation between serum TFF2 and hs-CRP,TNF-α,IL-18,APACHE Ⅱ scores in AP patients(r=0.496,0.541,0.474,0.418,P<0.05);miR-186-5p was negatively correlated with hs-CRP,TNF-α,IL-18,and APACHE Ⅱ scores(r=-0.421,-0.561,-0.443,-0.425,P<0.05).ROC analysis showed that the area under the curve(AUC)of TFF2 and miR-186-5p to assist in assessing whether severe AP occurred was 0.800(95%CI:0.717-0.882)and 0.824(95%CI:0.747-0.900)respectively,the sensitivity,specificity,and AUC of the combined detection were 80.00%,83.30%,and 0.888(95%CI:0.826-0.950),respectively.Multivariate Logistic regression analysis showed that TFF2 was a risk factor for severe AP,and m
