摘  要:目的观察双眼视感知觉训练对外伤性外旋转斜视矫正术后患者视功能的影响。方法前瞻性随机对照研究。纳入郑州大学第一附属医院2020年1月至2023年1月外伤性外旋转斜视90例作为研究对象,所有患者均行斜视矫正术,于术后1周采用随机数字表法将患者分成两组,每组45例。对照组术后行传统视觉训练干预;观察组术后行双眼视感知觉训练干预。对两组患者的临床疗效、视功能指标、三级视功能恢复、视力恢复时间、不良反应进行对比观察。结果观察组患者有效率为88.89%(21/45),显著优于对照组的68.89%(14/45)(χ^(2)=5.40,P=0.020)。视野检查仪显示观察组平均缺损值(4.66±2.14)dB和视野平均光敏感度(23.89±2.08)dB均低于对照组的(5.97±1.12)、(25.17±2.26)dB(t=3.64、2.80,P=0.001、0.006)。观察组中Ⅰ级视功能恢复率为37.78%(17/45),低于对照组的15.56%(7/45)(χ^(2)=0.07,P=0.968);观察组Ⅲ级视功能恢复率为62.22%(28/45),高于对照组的40.00%(18/45)(χ^(2)=6.34,P=0.042)。观察组视力恢复时间为(11.4±3.3)d,低于对照组的(18.5±3.6)d(t=8.34,P<0.001)。观察组不良反应的发生率为24.44%(11/45),低于对照组的44.44%(20/45)(χ^(2)=3.99,P=0.046)。结论双眼视感知觉训练能显著提高外伤性外旋转斜视患者的视功能,缩短患者的视力恢复时间,减缓其疾病进展,且治疗安全性较高。Objective To observe the efficacy of binocular visual perception training for postoperative patients with traumatic excyclotropia.Methods This was a prospective randomized controlled study.A total of 90 patients with traumatic exotropia in the First Affiliated Hospital of Zhengzhou University from Jan.2020 to Jan.2023 were included.All cases underwent strabismic surgery.The patients were divided into two groups with 45 cases in each group by random number table method at 1 week after surgery.The observation group received binocular visual perception training nursing intervention after operation.The control group received traditional visual training nursing intervention after operation.The clinical efficacy,visual function index,tertiary visual function recovery,visual recovery time and adverse reaction index between the two groups were compared and observed.Results The effective rate of the observation group was 88.89%(21/45),which was significantly better than the 68.89%(14/45)of the control group(χ^(2)=5.40,P=0.020).The visual field mean defect value(4.66±2.14)dB and the mean light sensitivity(23.89±2.08)dB in the observation group were lower than those in the control group[(5.97±1.12)and(25.17±2.26)dB](t=3.64,2.80;P=0.001,0.006).The recovery rate of grade I visual function in the observation group was 37.78%(17/45),which was lower than the 15.56%(7/45)in the control group(χ^(2)=0.07,P=0.968).The recovery rate of grade III visual function in the observation group was 62.22%(28/45),which was higher than the 40.00%(18/45)in the control group(χ^(2)=6.34,P=0.042).The visual acuity recovery time of the observation group was(11.4±3.3)days,which was lower than the(18.5±3.6)days of the control group(t=8.34,P<0.001).The incidence of postoperative adverse reaction in the observation group was 24.44%(11/45),which was lower than the 44.44%(20/45)in the control group(χ^(2)=3.99,P=0.046).Conclusion Binocular visual perception training can significantly improve the visual function of patients with traumatic external
