板栗刺角膜异物58例的临床特点及治疗效果观察

摘　　要：目的探讨板栗刺导致角膜异物的临床特点及治疗效果。方法回顾性病例系列研究。对南阳市眼科医院2015年9月至2022年10月住院的板栗刺角膜异物患者的病历资料进行分析。入选患者随访3个月,分析患者的一般资料、病变部位、病变特点和治疗方式。结果58例中受伤至入院时间最长7个月,最短4 h。单根角膜栗刺异物15例,多根角膜栗刺异物43例。角膜异物伴巩膜及结膜下异物9例,角膜穿孔伤47例,外伤性白内障5例,前房积脓2例。所有患者入院后均以1%伏立康唑联合抗生素等滴眼液滴眼。1次异物取出者51例,多次异物取出者7例。外伤性白内障摘出人工晶状体植入术5例。经过药物及手术治疗,术后随访3个月,患者眼部感染均得到控制。结论板栗刺所致的眼部外伤多引起睑裂区角膜或巩膜多发性异物,及时取出异物,应用伏立康唑联合抗生素滴眼液滴眼,是治疗细菌及真菌性角膜炎的有效方法。Objective To investigate the clinical characteristics and efficacy of corneal foreign body injury caused by chestnut thorn.Methods This was a retrospective case series study.Data of patients who had corneal foreign body injury by chestnut stab hospitalized in Nanyang Municipal Eye Hospital from Sep.2015 to Oct.2022 were collected.All patients were followed up for 3 months.General information,lesion region,and characteristics and treatment methods were analyzed.Results A total of 58 hospitalized patients were collected.The maximum time of injury to admission was 7 months and the minimum was 4 hours.There were 15 cases of single and 43 cases of multiple corneal chestnut thorn foreign bodies.There were 9 cases of corneal foreign body accompanied by scleral and subconjunctival foreign body,47 cases of corneal perforation injuries,5 cases of traumatic cataract and 2 cases of anterior chamber pus accumulation.All patients were given 1%voriconazole combined with antibiotics and other eye drops.There were 51 cases of single foreign body removal and 7 cases of multiple foreign body removal.There were 5 cases of traumatic cataract extraction intraocular lens implantation.The infections of 58 patients were controlled after 3 months of postoperative follow-up.Conclusion Multiple foreign bodies in cornea or sclera in palpebral fissure area are often caused by ocular trauma from chestnut spines.It is an effective form of treatment for the occurrence of bacterial and fungal keratitis by removing foreign bodies in time and applying voriconazole and antibiotic eye drops.
