摘  要:肩关节半脱位(GHS)是脑卒中后患侧上肢运动功能障碍的常见并发症之一,目前其发病机制未完全明确,尚无统一的临床康复治疗标准,对脑卒中后患者上肢运动功能的恢复造成了极大阻碍。近年来,随着对GHS生物力学机制的深入研究,以改善力学失衡为目标的中西医康复策略逐步增多,传统康复的针灸、推拿疗法及传统功法,现代康复的运动疗法、物理因子治疗、上肢康复机器人及3D打印技术等以肩关节周围组织力学结构的重塑和恢复为目的的康复策略均疗效显著。进一步明确GHS的发病机制、关注其长期疗效及多中心、大样本的临床康复治疗是未来的研究方向。Glenohumeral subluxation(GHS)is one of the common complications of upper extremity motor dysfunction after stroke.The pathogenesis is not yet completely clear,and there is no unified clinical rehabilitation treatment standard,which greatly hinders the rehabilitation process of the patients.In recent years,with the in-depth research on the biomechanical mechanism of GHS,the Chinese and western medicine rehabilitation strategies aiming at improving the mechanical imbalance have gradually increased.Rehabilitation strategies aimed at reshaping and restoring the mechanical structure of the tissues around the shoulder joint,such as traditional rehabilitation therapies like acupuncture,tuina therapy and traditional gong methods,and modern rehabilitation therapies like exercise therapy,physical factor therapy,upper limb rehabilitation robots and 3D printing technology,are all highly effective.Further clarification of the pathogenesis,attention to long-term efficacy and clinical rehabilitation treatment with multicenter and large samples are the directions of the future research.
