摘 要:未分化/去分化子宫内膜癌是临床罕见的高侵袭性肿瘤,预后差,诊断时往往处于晚期,且对常规化疗耐药。因此,新的治疗策略亟待探索。未分化/去分化子宫内膜癌具有不同于常见子宫内膜癌的特殊遗传学特征。其肿瘤发生通路具有独有的特征,如微卫星不稳定/错配修复蛋白、开关/蔗糖非发酵复合物染色质重塑复合物的基因失活。因此分子检测对未分化/去分化子宫内膜癌的临床管理、参与临床试验和靶向治疗具有重要意义。本文总结了未分化/去分化子宫内膜癌这两个罕见亚型的分子特征和临床研究,以探索当前和未来研究的潜在分子靶点。Undifferentiated/dedifferentiated endometrial carcinoma is rare and highly aggressive tumor with poor prognosis.It is often diagnosed at an advanced stage and resistant to conventional chemotherapy.Therefore,new treatment strategies need to be explored urgently.Undifferentiated/dedifferentiated endometrial carcinoma has genetic characteristics that are different from other common endometrial carcinomas.Its tumorigenic pathways have unique features,such as gene inactivation of microsatellite instability/mismatch repair proteins,and switch in mating type/sucrose non fermentation chromatin remodeling complexes.Therefore,molecular detection is of great significance for the clinical management of undifferentiated/dedifferentiated endometrial carcinoma,and participation in clinical trials and targeted therapy.In this review,the molecular features and clinical studies on those two uncommon subtypes are summarized in order to identify potential molecular targets for current and future research.
