47例酒精戒断震颤谵妄合并肺部感染患者药物治疗的临床分析

摘　　要：目的:探讨酒精戒断震颤谵妄合并肺部感染患者临床表现、治疗及转归情况,为临床治疗此类疾病提供一定的参考依据。方法:对2019年1月至2021年12月入住绵阳市第三人民医院的47例酒精戒断震颤谵妄合并肺部感染患者的病历资料进行回顾性分析。分析对症给药法合并抗精神病药物治疗酒精戒断综合征的疗效。结果:47例患者中,静脉使用地西泮最大日剂量110.5(60.4±28.1)mg,有38例合并使用氟哌啶醇和/或奥氮平;28例联合使用两种及以上抗生素;患者平均住院时间12.3(10.3±5.2)d;戒断症状基本控制时间为115.2(79.2±62.4)h。完全康复44例,死亡或者放弃治疗3例。结论:静滴地西泮对症给药法合并小剂量抗精神病药物对酒精戒断震颤谵妄合并肺部感染患者的治疗是相对安全和有效的。Objective:To study the clinical manifestations,treatment,and outcome of alcohol withdrawal delirium(delirium tremens)in patients with lung infection.Methods:The clinical data of 47 patients who suffered from delirium tremens with lung infection were retrospectively analyzed from January 2019 to December 2021 in the Third People's Hospital of Mianyang.All the patients received symptom-triggered treatment combined with antipsychotic medication.Results:Of the 47 patients,the maximum daily intravenous diazepam dose was 110.5(60.4±28.1)mg.There were 38 patients with the combined use of haloperidol and/or olanzapine,the length of stay in the hospital was 12.3(10.3±5.2)d,and 28 patients received two or more antibiotics in combination.The time to remis-sion of severe withdrawal symptoms was 115.2(79.2±62.4)h.A total of 44 patients were fully re-covered,and 3 patients died or gave up treatment.Conclusion:Intravenous diazepam treatment com-bined with a small dose of haloperidol and/or olanzapine is relatively safe and effective in patients who are affected by delirium tremens with lung infection.
