中医穴位敷贴联合针灸预防深静脉置管后血栓形成临床观察

摘　　要：目的:研究中医穴位敷贴联合针灸在预防深静脉置管后血栓形成的效果。方法:选取2022年7月至2023年6月南宁市第二人民医院行深静脉置管的60例患者,随机数字表法分为两组,每组30例。置管后对照组患者给予常规护理治疗,观察组患者在对照组的基础上增加中医穴位敷贴联合针灸,比较两组患者凝血功能、疼痛程度、患肢肿胀发生情况以及深静脉超声结果。结果:置管后第7天,观察组患者血浆凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)水平长于对照组,D–二聚体低于对照组,差异具有统计学意义(P <0.05);置管后第7天观察组疼痛程度患者低于对照组,差异具有统计学意义(P <0.05);置管后第7天,观察组患者患肢肿胀发生率低于对照组,差异具有统计学意义(P <0.05);置管后第7天,观察组患者深静脉血栓形成发生率低于对照组,差异具有统计学意义(P <0.05)。结论:在患者深静脉置管后予以中医穴位敷贴联合针灸,能够改善患者凝血功能,减轻肢体肿胀和疼痛程度,有效预防深静脉血栓形成的发生。Objective To study the effect of Traditional Chinese Medicine(TCM) acupoint application combined with acupuncture in preventing thrombosis after deep vein catheterization.Methods A total of 60 patients who underwent deep vein catheterization at Nanning Second People's Hospital from July 2022 to June 2023 were selected and randomly divided into two groups,with 30cases in each group.Patients in the control group received routine nursing care after catheterization,while patients in the observation group were additionally treated with TCM acupoint application combined with acupuncture.Coagulation function,pain level,incidence of limb swelling,and deep vein ultrasound results were compared between the two groups.Results On the 7th day after catheterization,the prothrombin time(PT) and activated partial thromboplastin time(APTT) levels in the observation group were significantly longer than those in the control group,and the D-dimer level was lower,with statistically significant differences(P < 0.05).The pain level in the observation group was significantly lower than that in the control group on the 7th day after catheterization(P < 0.05).The incidence of limb swelling in the observation group was significantly lower than that in the control group on the 7th day after catheterization(P < 0.05).The incidence of deep vein thrombosis in the observation group was significantly lower than that in the control group on the 7th day after catheterization(P < 0.05).Conclusion The application of TCM acupoint application combined with acupuncture after deep vein catheterization can improve patients' coagulation function,reduce limb swelling and pain level,and effectively prevent the occurrence of deep vein thrombosis.
