单眼慢性泪囊炎患者鼻中隔偏曲的情况观察

摘　　要：目的基于泪道电子计算机断层扫描(CT)造影观察单眼慢性泪囊炎患者鼻中隔偏曲情况。方法纳入2019年7月—2021年2月于成都中医药大学附属医院眼科就诊的单眼慢性泪囊炎患者181例。所有患者入院后均进行泪道CT造影检查,观察鼻中隔偏曲情况,并测量泪囊顶部与底部到鼻中隔的垂直距离。结果(1)鼻中隔偏曲情况:有140例(77.35%)患者存在鼻中隔偏曲,其中鼻中隔偏向患侧的有113例(80.71%),鼻中隔偏向健侧的有27例(19.29%)。(2)泪囊顶部与底部到鼻中隔的垂直距离分析:患侧泪囊顶部到鼻中隔的平均垂直距离低于健侧泪囊顶部到鼻中隔的平均垂直距离,差异有统计学意义(t=-9.293,P=0.000);患侧泪囊底部到鼻中隔的平均垂直距离低于健侧泪囊底部到鼻中隔的平均垂直距离,差异有统计学意义(t=-9.238,P=0.000)。结论本研究发现单眼慢性泪囊炎多见于中老年女性患者,多伴有鼻中隔偏曲,且患侧泪囊到鼻中隔的垂直距离小于健侧泪囊到鼻中隔的距离。OBJECTIVE To observe the nasal septal deviation in patients with unilateral chronic dacryocystitis using lacrimal duct computed tomography(CT)imaging.METHODS A total of 181 patients diagnosed with unilateral chronic dacryocystitis from July 2019 to February 2021 at Hospital of Chengdu University of Traditional Chinese Medicine were included.All patients underwent lacrimal duct computed tomography(CT)imaging upon admission to assess nasal septal deviation and measure the vertical distance from the top and bottom of the lacrimal sac to the nasal septum.RESULTS(1)Nasal septal deviation:Among the patients,140 cases(77.35%)exhibited nasal septal deviation,with 113 cases(80.71%)deviating toward the affected side and 27 cases(19.29%)deviating toward the healthy side.(2)Analysis of vertical distances from the top and bottom of the lacrimal sac to the nasal septum:The mean vertical distance from the top of the lacrimal sac to the nasal septum on the affected side was lower than that on the healthy side,with statistically significant differences(t=-9.293,P=0.000).Similarly,the mean vertical distance from the bottom of the lacrimal sac to the nasal septum on the affected side was lower than that on the healthy side,with statistically significant differences(t=-9.238,P=0.000).CONCLUSIONS This study revealed that unilateral chronic dacryocystitis is more common in middle-aged and elderly female patients and is often accompanied by nasal septal deviation.Additionally,the vertical distance from the lacrimal sac to the nasal septum on the affected side is smaller than that on the healthy side.
