镇肝熄风汤加减辅助治疗椎-基底动脉供血不足眩晕的效果

摘　　要：目的:探讨镇肝熄风汤加减辅助治疗椎-基底动脉供血不足(VBI)眩晕的效果。方法:选取2021年5月—2023年5月玉林市第二人民医院收治的VBI眩晕患者160例作为研究对象,按照随机数字表法分为观察组、对照组,各80例。对照组接受长春西汀联合天麻素治疗,观察组在对照组基础上接受镇肝熄风汤加减治疗。比较两组临床疗效、椎-基底动脉血流动力学、眩晕情况、血液流变学。结果:观察组总有效率高于对照组,差异有统计学意义(P=0.030);治疗后,观察组椎动脉、基底动脉的血管搏动指数均低于对照组,平均速度均高于对照组,差异有统计学意义(P<0.001);治疗后,两组眩晕评估量表评分均降低,观察组低于对照组,差异有统计学意义(P<0.05);治疗后,观察组纤维蛋白原、血细胞比容、全血黏度水平均低于对照组,差异有统计学意义(P<0.001)。结论:镇肝熄风汤加减辅助治疗VBI眩晕,不仅能够改善眩晕症状及脑部血流情况,还可降低血液黏度,提高临床疗效。Objective:To investigate the effect of the Zhengan Xifeng decoction addition and subtraction in the adjuvant treatment of vertebrobasilar artery blood supply insufficiency(VBI)vertigo.Methods:One hundred and sixty patients with VBI vertigo admitted to Yulin Second People's Hospital from May 2021 to May 2023 were selected as the study subjects,and they were divided into the observation group and the control group according to the method of randomized numerical table,each with eighty cases.The control group was treated with vincristine combined with gastrodin,and the observation group was treated with Zhengan Xifeng decoction addition and subtraction on the basis of the control group.The two groups were compared in terms of clinical efficacy,vertebrobasilar hemodynamics,vertigo,and blood rheology.Results:The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P=0.030);after treatment,the vascular pulsatility index of vertebral artery and basilar artery of the observation group was lower than that of the control group,and the average velocity was higher than that of the control group,and the difference was statistically significant(P<0.001);after treatment,the Vertigo Assessment Scale scores of both groups were reduced,and the observation group was lower than the control group,and the difference was statistically significant(P<0.05);after treatment,the levels of fibrinogen,hematocrit and whole blood viscosity of the observation group were lower than those of the control group,and the difference was statistically significant(P<0.001).Conclusion:The adjuvant treatment of VBI vertigo with Zhengan Xifeng decoction addition and subtraction can not only improve the symptoms of vertigo and cerebral blood flow,but also reduce the blood viscosity and improve the clinical efficacy.
