固定矫治器正畸联合种植义齿修复治疗牙列缺损合并错(牙合)畸形患者的效果

摘　　要：目的:观察固定矫治器正畸联合种植义齿修复治疗牙列缺损合并错(牙合)畸形患者的效果。方法:选取2019年6月至2021年6月该院收治的82例牙列缺损合并错(牙合)畸形患者进行前瞻性研究,按照掷硬币法分为观察组与对照组各41例。对照组予以种植义齿修复治疗,观察组在对照组基础上联合固定矫治器正畸治疗,比较两组治疗优良率、咀嚼效率、红色美学指数(PES)评分、白色美学指数(WES)评分和并发症发生率。结果:观察组治疗优良率为95.12%,明显高于对照组的80.49%,差异有统计学意义(P<0.05);修复治疗后3个月,两组咀嚼效率高于治疗前,且观察组高于对照组,差异均有统计学意义(P<0.05);修复治疗后3个月,观察组PES、WES评分均高于对照组,差异有统计学意义(P<0.05);观察组并发症发生率为4.88%,明显低于对照组的24.39%,差异有统计学意义(P<0.05)。结论:固定矫治器正畸联合种植义齿修复治疗牙列缺损合并错(牙合)畸形患者可提高治疗优良率、咀嚼效率、PES和WES评分,以及降低并发症发生率,效果优于单纯种植义齿修复治疗。Objective:To observe effects of fixed appliance orthodontic combined with implant denture repair in treatment of patients with dentition defects and malocclusion.Methods:A prospective study was conducted on 82 patients with dentition defects and malocclusion admitted to the hospital from June 2019 to June 2021.According to the coin tossing method,they were divided into observation group and control group,41 cases in each group.The control group was treated with implant denture repair,while the observation group was treated with fixed appliance orthodontic treatment on the basis of that of the control group.The excellent and good rate of treatment,the masticatory efficiency,the pink esthetic index(PES)score,the white aesthetic index(WES)score,and the incidence of complications were compared between the two groups.Results:The excellent and good rate of treatment in the observation group was 95.12%,which was significantly higher than 80.49%in the control group,and the difference was statistically significant(P<0.05).Three months after repair treatment,the masticatory efficiencies of the two groups were higher than those before the treatment,that of the observation group was higher than that of the control group,and the differences were statistically significant(P<0.05).Three months after repair treatment,the PES,WES scores of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).Further,the incidence of complications in the observation group was 4.88%,which was significantly lower than 24.39%in the control group,the difference was statistically significant(P<0.05).Conclusions:Fixed appliance orthodontics combined with implant denture repair for the patients with dentition defects and malocclusion can improve the excellent and good rate of treatment,the masticatory efficiency,the PES and WES scores,and reduce the incidence of complications.Moreover,it is superior to single implant denture repair.
