急性牙髓炎实施常温流动牙胶根管治疗的临床应用价值探讨

摘　　要：目的探讨常温流动牙胶根管治疗在急性牙髓炎中的治疗价值。方法选取2019.03到2021.03期间本院收治的96例急性牙髓炎患者分别纳入观察组、对照组,各48例。对照组以热牙胶充填+iRoot糊剂治疗,观察组增加常温流动牙胶治疗。评估比较两组患者临床疗效、填充效果、疼痛程度、炎症因子水平和治疗安全性等指标。结果观察组总有效率为91.67%(44/48),高于对照组为72.92%(35/48)(P<0.05)。观察组治疗后1d、4d、7d时VAS评分均低于对照组(P<0.05)。观察组恰填率高于对照组,填充时间低于对照组(P<0.05)。治疗后观察组RANKL、转录激活因子(STAT1)、基质金属蛋白抑制酶1(TIMP-1)低于对照组(P<0.05)。观察组不良反应发生率为8.33%(4/48),低于对照组27.08%(13/48)(P<0.05)。结论急性牙髓炎患者实施常温流动牙胶治疗有良好效果,显著降低患者治疗后的疼痛程度,改善患者的充填的恰填率,抑制患者术后的各种炎症因子RANKL、STAT1、TIMP-1表达水平,且能进一步降低患者的不良反应发生情况。Objective To investigate the value of root canal treatment with normal temperature flowing dental glue in acute pulpitis.Methods 96 patients with acute pulpitis admitted to our hospital from 2019.03~2021.03 observation,control group,with 48 patients.Control group treated with hot gum filling+iroot,observation group was treated with normal temperature flowing gum.The clinical efficacy,filling effect,pain degree,inflammatory factor level and treatment safety.Results observation group total effective rate was 91.67%(44/48),higher than control group 72.92%(35/48)(P<0.05).The observation group VAS scores were lower than control group on 1D,4D and 7d after treatment(P<0.05).The observation group filling rate was higher than control group,and the filling time was lower than that of the control group(P<0.05).The observation group levels of inflammatory factors including osteoclast differentiation factor(RAN KL),transcription activator(STAT1)and matrix metalloproteinase inhibitor-1(TIMP-1)were lower than control group(P<0.05).Conclusion The application of normal temperature flow dental glue in the treatment of patients with acute pulpitis has a good effect,which can significantly reduce the pain degree of patients after treatment,improve the filling rate of patients,inhibit the expression levels of various inflammatory factors RANAN KL,STAT1,TIMP-1,and further reduce the incidence of adverse reactions in patients.
