摘　　要：目的:分析宫颈上皮内瘤变(CIN)患者的宫颈锥切特征与术后病变残留及HPV转阴之间的相关因素。方法:采用双中心回顾性研究分析507例CIN患者的基本资料、HPV状态、宫颈锥切手术特征以及术后结果。结果:患者的平均年龄为41.69±9.35岁。手术方式以LEEP为主(86.59%);术后病变残留率为7.89%。多因素逻辑回归分析揭示年龄、绝经、术前组织活检分级、锥切深度、内外切缘、腺体受累和术前多重HPV感染与病变残留显著关联(P<0.05);而锥高、腺体受累、术前多重HPV及高危HPV感染与HPV转阴显著关联(P<0.05)。结论:腺体受累和术前多重HPV感染等因素与CIN患者宫颈锥切术后病变残留及HPV转阴相关,为制定个性化治疗方案提供了参考。Objective:To analyze the factors related to cervical conization characteristics,postoperative lesion re⁃siduals,and HPV clearance in patients with Cervical Intraepithelial Neoplasia(CIN).Methods:A bicentric retrospec⁃tive study was conducted,analyzing the basic information,HPV status,cervical conization surgery characteristics,and postoperative outcomes of 507 CIN patients.Results:The mean age of the patients was 41.69±9.35 years,with LEEP be⁃ing the primary surgery(86.59%).The postoperative lesion residual rate was 7.89%.Multivariate logistic regression anal⁃ysis revealed significant associations between lesion residuals and factors such as age,menopausal,preoperative tissue biopsy,cone depth,margins,gland involvement,and preoperative multiple HPV infections(P<0.05).Similarly,cone height,gland involvement,preoperative multiple HPV and high⁃risk HPV were significantly related to HPV clearance(P<0.05).Conclusion:Factors like gland involvement and preoperative multiple HPV infections are associated with the residuals of lesions post⁃cervical conization surgery and HPV clearance in CIN patients,providing valuable insights in developing personalized treatment plans.
