超声乳化治疗急性闭角型青光眼合并白内障的效果分析

摘　　要：目的分析超声乳化治疗急性闭角型青光眼合并白内障的效果。方法随机选取2020年6月—2023年2月徐州复兴眼科医院收治的90例急性闭角型青光眼合并白内障患者为研究对象,以随机数表法分为对照组(45例,青光眼白内障联合手术+人工晶体置入术治疗)、观察组(45例,超声乳化白内障吸出术+人工晶体置入术治疗),对比两组治疗效果、临床指标及并发症。结果术后1周、1个月,观察组裸眼视力为(0.63±0.20)、(0.85±0.22)D,较对照组的(0.41±0.13)、(0.68±0.21)D更高,差异有统计学意义(t=6.187、3.750,P均<0.05);眼压为(15.58±2.24)、(12.50±2.18)mmHg,较对照组的(19.86±3.51)、(16.23±3.31)mmHg更低,差异有统计学意义(t=6.895、6.313,P均<0.05);观察组中央角膜厚度、前房深度较对照组更高,晶状体厚度与眼轴长度系数、晶状体混浊程度评分较对照组更低,差异有统计学意义(P均<0.05);观察组并发症总发生率(4.44%)较对照组(20.00%)更低,差异有统计学意义(χ^(2)=5.075,P<0.05)。结论超声乳化白内障吸除术治疗急性闭角型青光眼合并白内障能够显著提高术后裸眼视力,降低术后眼压,改善临床指标,降低并发症,从而加快康复进程。Objective To analyze the effect of phacoemulsification in treating acute angle-closure glaucoma with cataract.Methods A total of ninety patients with acute angle-closure glaucoma combined with cataract admitted to Xuzhou Fuxing Eye Hospital from June 2020 to February 2023 were randomly selected as the study objects.They were divided into control group(45 cases,glaucoma cataract combined operation+intraocular lens implantation)and observation group(45 cases,phacoemulsification cataract extraction+intraocular lens implantation)according to random number table method.The therapeutic effect,clinical indexes and complications were compared between the two groups.Results One week and one month after surgery,the naked eye visual acuity of observation group were(0.63±0.20)D,(0.85±0.22)D,which were higher than those of control group(0.41±0.13)D,(0.68±0.21)D,and the differences were statistically significant(t=6.187,3.750,both P<0.05).The intraocular pressure were(15.58±2.24)mmHg,(12.50±2.18)mmHg,which were lower than those of the control group(19.86±3.51)mmHg,(16.23±3.31)mmHg,and the differences were statistically significant(t=6.895,6.313,both P<0.05).The central corneal thickness and anterior chamber depth of the observation group were higher than those of the control group,and the lens thickness and axial length coefficient and the lens opacity score were lower than those of the control group,and the differences were statistically significant(all P<0.05).The total complication rate of the observation group was 4.44%,which was lower than that of the control group 20.00%,and the difference was statistically significant(χ^(2)=5.075,P<0.05).Conclusion Phacoemulsification in the treatment of acute angle-closure glaucoma combined with cataract can significantly improve the postoperative naked eye visual acuity,reduce postoperative intraocular pressure,improve clinical indicators,reduce complications,and thus accelerate the rehabilitation process.
