摘　　要：目的 探讨动态增强CT扫描诊断胃癌的效能及其术前TNM分期与术后病理检查结果的一致性。方法 选取邓州市中心医院2021年10月至2023年7月收治的经术后病理活检证实为胃癌患者87例,统计胃癌患者动态增强CT征象及分期结果,分析动态增强CT对胃癌的诊断价值,分析动态增强CT扫描对胃癌术前TNM分期的诊断与术后病理诊断的一致性。结果 87例胃癌患者中,动态增强CT扫描检出胃癌患者85例,胃癌检出率为97.70%,漏诊率为2.30%;动态增强CT扫描与术后病理检查分型结果的一致性Kappa值为0.933;动态增强CT扫描胃癌术前T、N及TNM期与术后病理检查结果的一致性分析,Kappa值分别为0.917、0.906、0.908,均>0.75,具有较高的一致性。结论 动态增强CT扫描可显著提高胃癌检出率,降低漏诊率,其术前TNM分期与术后病理检查结果一致性较高,具有较高的临床应用价值。Objective To explore the efficacy of dynamic enhanced CT scanning in the diagnosis of gastric cancer and the consistency between preoperative TNM staging and postoperative pathological examination results.Methods 87 patients with gastric cancer confirmed by postoperative pathological biopsy admitted to Dengzhou Central Hospital from October 2021 to July 2023 were selected.The dynamic enhanced CT signs and staging results of gastric cancer patients were collected,and the diagnostic value of dynamic enhanced CT for gastric cancer was analyzed.The consistency between the preoperative TNM staging and postoperative pathological diagnosis of gastric cancer was analyzed.Results Among 87 gastric cancer patients,85 were detected by dynamic enhanced CT scanning,with a detection rate of 97.70%and a missed diagnosis rate of 2.30%;The consistency between dynamic contrast-enhanced CT scan and postoperative pathological examination classification results was a Kappa value of 0.933;The consistency analysis between preoperative T,N,TNM phases and postoperative pathological examination results of dynamic enhanced CT scanning for gastric cancer showed that Kappa values were 0.917,0.906,and 0.908,all>0.75,indicating high consistency.Conclusion Dynamic enhanced CT scanning can significantly improve the detection rate of gastric cancer and reduce the missed diagnosis rate.Its preoperative TNM staging is consistent with postoperative pathological examination results,and has high clinical application value.
