摘　　要：目的探讨微小RNA-10a-5p(miR-10a-5p)、微小RNA-217-5p(miR-217-5p)、微小RNA-370-3p(miR-370-3p)水平联合检测对急性髓系白血病(acute myeloid leukemia,AML)患者复发的预测价值。方法 回顾性选取2021年6月至2022年12月西安国际医学中心医院收治的AML患者78例,纳入观察组,根据1∶1选例原则,另选取同期单纯贫血患者78例,纳入对照组。比较两组、不同疗效及不同复发情况患者骨髓细胞miR-10a-5p、miR-217-5p、miR-370-3p水平,分析各指标水平与疗效及复发的关系。结果 观察组miR-10a-5p水平高于对照组,miR-217-5p、miR-370-3p水平低于对照组(P<0.05);不同疗效患者miR-10a-5p水平比较:未缓解>部分缓解>完全缓解,miR-217-5p、miR-370-3p水平比较:未缓解<部分缓解<完全缓解(P<0.05);复发患者miR-10a-5p水平高于未复发患者,miR-217-5p、miR-370-3p水平低于未复发患者(P<0.05);AML患者miR-10a-5p水平与疗效及复发情况均呈正相关,miR-217-5p、miR-370-3p水平与疗效及复发情况均呈负相关,且联合预测AML患者复发AUC为0.913,约登指数为12.32,最佳诊断敏感度为84.21%,特异度为88.14%。结论 miR-10a-5p、miR-217-5p、miR-370-3p水平与AML患者疗效及复发均具有一定相关性,联合检测对复发具有一定预测价值,可作为临床评估疗效及复发情况的辅助指标。Objective To explore the predictive value of combined detection of miR-10a-5p,miR-217-5p,and miR-370-3p levels for recurrence in patients with acute myeloid leukemia(AML).Methods 78 AML patients admitted to Xi'an International Medical Center Hospital from June 2021 to December 2022 were retrospectively selected and included in the observation group.According to the 1:1 selection principle,another 78 patients with simple anemia during the same period were selected and included in the control group.The levels of miR-10a-5p,miR-217-5p,and miR-370-3p in bone marrow cells of two groups of patients with different therapeutic effects and recurrence conditions were compared,and the relationship between the levels of each indicator and the therapeutic effect and recurrence was analyzed.Results The levels of miR-10a-5p in the observation group were higher than those in the control group,while the levels of miR-217-5p and miR-370-3p were lower than those in the control group(P<0.05);Comparison of miR-10a-5p levels in patients with different therapeutic effects:no response>partial response>complete response.Comparison of miR-217-5p and miR-370-3p levels:no response<partial response<complete response(P<0.05);The levels of miR-10a-5p in recurrent patients were higher than those in non recurrent patients,while the levels of miR-217-5p and miR-370-3p were lower than those in non recurrent patients(P<0.05);The levels of miR-10a-5p in AML patients were positively correlated with efficacy and recurrence,while the levels of miR-217-5p and miR-370-3p were negatively correlated with efficacy and recurrence.The combined prediction AUC for recurrence in AML patients was 0.913,the Youden index was 12.32,the optimal diagnostic sensitivity is 84.21%,and the specificity was 88.14%.Conclusion The levels of miR-10a-5p,miR-217-5p,and miR-370-3p are correlated with the efficacy and recurrence of AML patients.Combined detection has certain predictive value for recurrence and can be used as an auxiliary indicator for clinical evaluation of effica
