摘　　要：目的比较直出式绿激光膀胱肿瘤剜除术与经尿道膀胱肿瘤等离子电切术治疗非肌层浸润性膀胱癌(NMIBC)的疗效。方法回顾性分析2019年3月至2020年3月于安庆市立医院泌尿外科治疗的80例NMIBC患者的临床资料,通过随机数表法将80例患者均分为两组,其中实验组采用直出式绿激光膀胱肿瘤剜除术,对照组采用经尿道膀胱肿瘤等离子电切术。对两组的术中出血量、术后膀胱冲洗时间、留置尿管时间、术后住院时间以及术中、术后并发症发生情况进行比较分析。结果两组患者手术均顺利完成,实验组在手术时间、复发率方面与对照组相比差异无统计学意义(P>0.05)。实验组术中出血量、术后膀胱冲洗时间、留置尿管时间、术后住院时间小于对照组;实验组在术后出血、闭孔神经反射和膀胱穿孔发生率明显低于对照组,差异有统计学意义(P<0.05)。结论直出式绿激光膀胱肿瘤剜除术治疗NMIBC较经尿道膀胱肿瘤等离子电切术更安全、有效,具有微创、出血少、并发症风险低等优点,值得临床推广使用。Objective To compare the efficacy in the treatment of non-muscle invasive bladder cancer(NMIBC),between front-firing green-light laser enucleation and transurethral plasmakinetic resection.Methods The clinical data of 80 NMIBC patients treated in the urology department of Anqing Municipal Hospital from March 2019 to March 2020 were retrospectively analyzed.These 80 patients were divided into two groups by random number table method,the experimental group were treated with front-firing green-light laser and 40 patients in the control group were treated with transurethral plasma resection of bladder tumor.The intraoperative blood loss,postoperative bladder irrigation time,catheter indwelling time,postoperative hospital stay and the occurrence of intraoperative and postoperative complications were compared between the two groups.Results The operations were successfully completed in both groups.There was no significant difference in operation time or recurrence rate between the experimental group and the control group(P>0.05).The amount of intraoperative blood loss,postoperative bladder irrigation time,catheter retention time and postoperative hospital stay in the experimental group were superior to those in the control group.In terms of surgical complications,the incidence of postoperative bleeding,obturator nerve reflex and bladder perforation in the experimental group was significantly lower than that in the control group,with statistical significance(P<0.05).Conclusions Compared with transurethral plasma resection of bladder tumor,the treatment of NMIBC by front-firing green-light laser is safer and more effective.It has the advantages of minimall invasion,less bleeding and low complication risk,so it is worthy of clinical promotion.
