摘 要:自身免疫性肝炎(autoimmune hepatitis,AIH)是一种由免疫介导的慢性肝脏疾病,酒精、药物、毒素等为其诱导因素,至今其发病机制尚未完全阐明,在治疗上尚以缓解症状、延缓疾病进展为主。由于其免疫特性,T细胞作为免疫应答反应的主要细胞之一引起了人们的关注。近些年来国内外学者通过对AIH免疫机制的深入研究发现Th17与Treg细胞在AIH中的发生发展中起着关键作用。本文从自身免疫反应出发,阐明Th17和Treg细胞除了其自身单独作用之外,在数量和功能方面还会互相影响,并且IL-17、IL-6、RORγt、FoxP3等相关细胞因子也可以通过影响Th17细胞和/或Treg细胞水平进而影响AIH的发生发展。Autoimmune hepatitis(AIH)as an immune-mediated chronic liver disease,influenced by factors such as alcohol,drugs,toxins and other triggers,yet its precise pathogenesis remains incompletely understood.Treatment primarily focuse on relieving symptoms and delaying disease progression.Due to their immune characteristics,T cells have garnered attention as pivotal cells of the immune response.Recent research,scholars at home and abroad have found that Th17 cells and Treg cells play key roles in the occurrence and development of AIH through in-depth research on the immune mechanism of AIH.Starting from the autoimmune response,this article elucidated that Th17 and Treg cells not only exert individual effects,but also intricately interact in terms of quantity and functionality.Furthermore,cytokines such as IL-17,IL-6,RORγt,FoxP3 and others can impact the development of AIH by affecting the levels of Th17 cells and/or Treg cells.
