低剂量螺旋CT仿真内镜技术在早期结直肠癌诊断中的筛查价值及准确性分析

摘　　要：目的:分析低剂量螺旋CT仿真内镜技术在早期结直肠癌诊断中的筛查价值及准确性。方法:选取2020年5月-2021年10月河南科技大学第一附属医院收治的160例疑似结直肠癌患者作为研究对象。对所有患者实施常规CT检查和低剂量螺旋CT仿真内镜技术检查,以病理检查结果为金标准,对比分析常规CT检查和低剂量螺旋CT仿真内镜技术检查的准确率。结果:经病理检查确诊有120例为恶性结直肠癌,另外40例为良性结直肠肿瘤;低剂量螺旋CT仿真内镜检查的灵敏度、特异度、良性预测值、恶性预测值、诊断准确率均明显高于常规CT检查,差异有统计学意义(χ^(2)=17.954、9.842、9.830、17.245、19.472,P<0.05);低剂量螺旋CT仿真内镜检查的DLP、CTDlw显著低于常规CT检查,差异有统计学意义(t=385.143、127.114,P<0.05)。结论:低剂量螺旋CT仿真内镜技术能够准确的诊断筛查早期结直肠癌,并能够获得比CT更高的诊断灵敏度、准确度、特异度,具有重要的诊断价值。Objective:To analyze the screening value and accuracy of low-dose spiral CT virtual endoscopy in the diagnosis of early colorectal cancer.Methods:160 suspected colorectal cancer patients in the hospital from May 2020 to October 2021 were included in the study.All patients were examined with conventional CT and low-dose spiral CT virtual endoscopy,with pathological examination results as the gold standard.The accuracy of conventional CT and low-dose spiral CT virtual endoscopy was compared and analyzed.Results:120 selected cases were diagnosed as malignant colorectal cancer by pathological examination.The other 40 cases were benign colorectal tumors.Specificity,sensitivity,benign predictive value,malignant predictive value of low-dose spiral CT virtual endoscopy were significantly higher than those of conventional CT(χ^(2)=17.954,9.842,9.830,17.245,19.472;P<0.05).DLP and CTDlw of low-dose spiral CT virtual endoscopy were significantly lower than those of conventional CT(t=385.143,127.114;P<0.05).Conclusion:Low-dose of spiral CT virtual endoscopy can accurately diagnose and screen early colorectal cancer.It can obtain higher diagnostic sensitivity,accuracy and specificity than CT,and has very important diagnostic value.
