摘 要:目的分析在脑膜瘤侵袭性评估中应用多模态磁共振成像检查的效果。方法回顾性分析2020年1月至2023年11月在我院确诊为脑膜瘤的88例患者的多模态磁共振成像检查资料,根据手术病理结果将无侵袭性情况的46例患者设为对照组,将存在侵袭性情况的42例患者设为观察组,对比2组患者的多模态磁共振成像检查相关参数,应用受试者工作特征曲线(ROC曲线)分析多模态磁共振检查相关参数评估脑膜瘤侵袭性的效能。结果观察组的胆碱(Cho)和Cho/肌酸(Cr)高于对照组(P<0.05),观察组的表观弥散系数(ADC)、相对表观弥散系数(rADC)、最大肿瘤血流量(TBF)、Cr、N-乙酰天门冬氨酸(NAA)和NAA/Cho低于对照组(P<0.05),2组的NAA/Cr相近(P>0.05)。ROC曲线分析发现,ADC、rADC和TBF以及瘤体Cr、Cho、Cho/Cr和NAA/Cho评估脑膜瘤侵袭性的效能良好,NAA/Cr评估脑膜瘤侵袭性的效能较差(P>0.05)。结论多模态磁共振成像检查在脑膜瘤侵袭性评估中具有良好诊断效能。Objective To analyze the effect of applying multimodal magnetic resonance imaging(MRI)examination in the assessment of invasiveness of meningioma.Methods To retrospectively analyze the multimodal magnetic resonance examination data of 88 patients diagnosed with meningiomas in our hospital from January 2020 to November 2023.According to the results of surgical pathology,46 patients without invasive conditions were set as the control group,and 42 patients with invasive conditions were set as the observation group.Compare the multimodal magnetic resonance examination-related parameters of the patients in the two groups,and analyze the multimodal magnetic resonance examination-related parameters of the patients in the two groups by applying the working characteristic curve of the subjects(ROC curve)was applied to analyze the efficacy of multimodal magnetic resonance examination related parameters in assessing the invasiveness of meningioma.Results Choline(Cho)and Cho/creatine(Cr)were higher in the observation group than in the control group(P<0.05).Apparent diffusion coefficient(ADC),relative apparent diffusion coefficient(rADC),maximal tumor blood flow(TBF),Cr,N-acetylaspartic acid(NAA),and NAA/Cho were lower in the observation group than in the control group(P<0.05),and NAA/Cr were similar(P>0.05).ROC curve analysis revealed that the efficacy of ADC,rADC and TBF as well as tumor Cr,Cho,Cho/Cr and NAA/Cho in assessing meningioma aggressiveness was good(P<0.05),and the efficacy of NAA/Cr in assessing meningioma aggressiveness was poor(P>0.05).Conclusion Multimodal magnetic resonance examination has good diagnostic efficacy in the assessment of meningioma aggressiveness.
