改良筋膜鞘悬吊联合提上睑肌缩短术治疗重度上睑下垂的效果观察

摘　　要：目的观察分析改良筋膜鞘悬吊与提上睑肌缩短术联合治疗重度上睑下垂的效果。方法于2019年1月~2023年12月期间,选取我院收治的重度上睑下垂患者85例,均接受改良筋膜鞘悬吊联合提上睑肌缩短术治疗,观察其治疗情况。结果术后3个月患者矫治良好58例,基本矫治24例,矫治欠佳3例,矫治率为96.47%。术后3个月患者睑裂高度(PFH)、上睑缘至角膜映光点距离(MRD1)水平较术后1周明显降低(P<0.05);术后3个月患者发生眼睑血肿2例,未发生结膜脱垂、暴露性角膜炎、睑缘弧度不佳等并发症,并发症发生率为2.35%。术后3个月满意度调查显示,满意率为96.47%。结论改良筋膜鞘悬吊联合提上睑肌缩短术效果确切,能有效改善PFH、MRD1,且术后并发症少,患者满意度高。Objective To observe and analyze the effect of modified fascial sheath suspension combined with levator muscle shortening in the treatment of severe blepharoptosis.Methods From September 2019 to December 2023,85 patients with severe upper eyelid ptosis admitted to our hospital were selected and treated with modified fascia sheath suspension combined with levator palpebrae muscle shortening surgery.The treatment status was observed.Results Three months after surgery,58 patients received good correction,24 received basic correction,and 3 had poor correction.The correction rate was 96.47%.The levels of eyelid fissure height(PFH)and distance from upper eyelid margin to corneal reflection point(MRD1)in patients at 3 months after surgery were significantly lower than those at 1 week after surgery(P<0.05);Three months after surgery,two patients developed eyelid hematoma without complications such as conjunctival prolapse,exposed keratitis,and poor eyelid curvature.The incidence of complications was 2.35%.The satisfaction survey conducted 3 months after surgery showed a satisfaction rate of 96.47%.Conclusion Modified fascial sheath suspension combined with levator muscle resection is effective,which can effectively improve PFH and MRD1,with fewer postoperative complications and high patient satisfaction.
