摘  要:目的 探究微小RNA-223(miR-223)在炎症性肠病(IBD)患者肠黏膜组织中的表达及与辅助性T细胞17(Th17)/调节性T细胞(Treg)的关系。方法 选择2021年1月—2023年11月本院收治的96例IBD患者作为研究对象,根据病情划分为活动期组(54例)和缓解期组(42例)两组;另选择同期51例结肠息肉者为对照组。比较三组miR-223、Th17细胞、Treg细胞水平及Th17/Treg细胞因子[白介素-17(IL-17)、IL-23、转化生长因子-β1(TGF-β1)、IL-10]水平;Pearson相关分析miR-223与Th17/Treg细胞及细胞因子的相关性。结果 miR-223水平活动期组(3.26±0.61)>缓解期组(2.41±0.52)>对照组(1.02±0.18)(P<0.05)。Th17细胞、Th17/Treg水平活动期组>缓解期组>对照组,Treg细胞水平活动期组<缓解期组<对照组,均P<0.05。IL-17、IL-23水平活动期组>缓解期组>对照组,TGF-β1、IL-10水平活动期组<缓解期组<对照组,均P<0.05。Pearson相关分析显示,miR-223与Th17、Th17/Treg、IL-17、IL-23呈正相关(r=0.711、0.742、0.760、0.751,均P<0.05);miR-223与Treg、TGF-β1、IL-10呈负相关(r=-0.685、-0.308、-0.850,均P<0.05)。结论 IBD患者肠黏膜组织中miR-223表达呈相对高水平,反映病情活动性,且与Th17/Treg细胞平衡有关。Objective To investigate the expression of microRNA-223(miR-223)in the intestinal mucosa of patients with inflammatory bowel disease(IBD)and its relationship with T helper cell 17(Th17)/regulatory T cell(Treg).Methods A total of 96 IBD patients who were admitted to our hospital from January 2021 to November 2023 were selected as the study subjects,and they were divided into the active stage group(54 cases)and the remission stage group(42 cases)according to their condition.Another 51 patients with colonic polyps were selected as the control group.The levels of miR-223,Th17 cells,Treg cells and Th17/Treg cytokines[interleukin-17(IL-17),IL-23,transforming growth factor-β1(TGF-β1),IL-10]were compared among the three groups.Pearson correlation analysis was used to analyze the correlation between miR-223 and Th17/Treg cells and cytokines.Results The difference in level of miR-223 in the active stage group(3.26±0.61),the remission stage group(2.41±0.52),and the control group(1.02±0.18)were statistically significant(P<0.05).The levels of Th17 cells and Th17/Treg in the active phase group,the remission phase group,and the control group were decreased one by one;the levels of Treg cells in the active phase group,the remission phase group,and the control group were increased one by one,all P<0.05.The levels of IL-17 and IL-23 in the active stage group,the remission stage group,and the control group decreased one by one,and the levels of TGF-β1 and IL-10 in the active stage group,the remission stage group,and the control group increased one by one,all P<0.05.Pearson correlation analysis showed that miR-223 was positively correlated with Th17,Th17/Treg,IL-17 and IL-23(r=0.711,0.742,0.760,0.751,all P<0.05).miR-223 was negatively correlated with Treg,TGF-β1 and IL-10(r=-0.685,-0.308,-0.850,all P<0.05).Conclusions The expression of miR-223 in the intestinal mucosa of IBD patients is relatively high,which reflects the disease activity and is related to the balance of Th17/Treg cells.
