粪便免疫化学试验与血清CA125、 ALP联合检测诊断结直肠癌的临床价值

摘　　要：目的:分析粪便免疫化学试验(FIT)与癌抗原125(CA125)、血清碱性磷酸酶(ALP)联合检测诊断结直肠癌的临床价值。方法:将2022年1月至2023年3月于我院确诊为结直肠癌患者62例设为观察组,另选同期75例无结直肠癌患病风险的志愿者设为对照组,并收集2组临床资料。对2组FIT、血清CA125、ALP的阳性率进行比较,对观察组患者FIT、血清CA125、ALP阳性率与不同病理特征的关系进行分析,就研究数据绘制ROC曲线分析该3种指标联合诊断结直肠癌的效能。结果:观察组患者FIT、血清CA125、ALP阳性率分别为100.00%、98.39%和96.77%,均高于对照组(P<0.05);结直肠癌临床分期越高,肿瘤直径越大,且伴有肝转移的患者FIT、血清CA125、ALP阳性率越高(P<0.05);不同肿瘤分型的阳性率比较无显著性差异(P>0.05);结直肠癌位于直肠的患者FIT、血清CA125、ALP阳性率与结直肠癌位于左、右半结肠的患者比较差异无统计学意义(P>0.05);经ROC曲线分析,FIT、血清CA125、ALP联合诊断结直肠癌的AUC为0.840,高于单独诊断,灵敏度为98.4%、特异度为68.0%。结论:FIT与血清CA125、ALP在结直肠癌患者中表达水平较高,联合诊断结直肠癌的效能较好,临床价值较高。Objective To analyze the clinical value of fecal immunochemical test(FIT)combined with cancer antigen 125(CA125)and serum alkaline phosphatase(ALP)in the diagnosis of colorectal cancer.Methods Total 62patients diagnosed with colorectal cancer in our hospital from January 2022to March 2023were set as the observation group,and 75volunteers without the risk of colorectal cancer in the same period were set as the control group,and the clinical data of the two groups were collected.The positive rates of FIT,serum CA125and ALP in the two groups were compared,and the relationship between the positive rates of FIT,serum CA125and ALP in the observation group and the different pathological char-acteristics was analyzed.The ROC curve was drawn to analyze the efficacy of the three indicators in the di-agnosis of colorectal cancer.Results The positive rates of FIT,serum CA125and ALP in the observation group were 100.00%,98.39%and 96.77%,respectively,which were higher than those in the control group(P<0.05).The higher the clinical stage of colorectal cancer,the larger the tumor diameter,and the higher the positive rates of FIT,serum CA125and ALP in patients with liver metastasis(P<0.05).There was no significant difference in the positive rates of different tumor types(P>0.05).There was no significant difference in the positive rate of FIT,serum CA125and ALP between patients with tumor located in the rectum and patients with tumor located in the left or right hemicolon(P>0.05).ROC curve analysis showed that the AUC of combined FIT,serum CA125and ALP in diagnosis of colorectal cancer was 0.840,higher than that of single diagnosis,with a sensitivity of 98.4%and a specificity of 68.0%.Conclusion The expression levels of FIT,serum CA125and ALP in colorectal cancer patients are higher,and the com-bined diagnosis of colorectal cancer has better efficacy and higher clinical value.
