摘　　要：目的探讨亚麻醉剂量氯胺酮对老年髋关节置换术后患者的认知功能及情绪的影响。方法本研究对象为2018年1月~2022年12月84例髋部疾病患者,均采用髋关节置换治疗,数字表法随机分为两组。对照组采用常规麻醉,观察组联合亚麻醉剂量氯胺酮,比较两组患者简易精神状态检查量表(Simple mental state examination scale,MMSE)、医院焦虑抑郁量表,比较两组患者认知功能障碍、谵妄发生情况。结果两组术后1 d、3d MMSE评分均低于术前,焦虑、抑郁评分均高于术前,且术后1 d、3 d观察组各项数据优于对照组(P<0.05);观察组并发症发生率显著低于对照组(P<0.05)。结论亚麻醉剂量氯胺酮能够降低老年髋关节置换术后患者的认知功能损害,减轻焦虑、抑郁情绪,降低并发症发生率。Objective To investigate the effect of bolus subanesthetic dose ketamine on postoperative cognitive function and mood in elderly hip replacements patients.Methods Eighty-four patients with hip disease from January 2018 to December 2022 were the subjects of this study,and all of whom were treated with hip joint replacement.They were randomly divided into two groups using the number table method.The control group received conventional anesthesia,and the observation group received subanesthetic dose of ketamine.The patients in the two groups were compared with the Simple Mental State Examination Scale(MMSE)and the Hospital Anxiety and Depression Scale before surgery.To compare the occurrence of cognitive dysfunction and delirium in the two groups.Results The MMSE scores were lower than the preoperative scores,and the anxiety,depression scores were higher than the preoperative scores in both groups 1d and 3d after surgery,and the data in the observation group were better than those in the control group ld and 3d after surgery(P<0.05).The complication in the observation group was significantly lower than that in the control group(P<0.05).Conclusion bolus subanesthetic dose ketamine can reduce cognitive impairment,reduce anxiety and depression,and reduce the incidence of complication after hip arthroplasty in the elderly.
