摘 要:目的运用力矩平衡理论与技术重建微小型唇裂美学形态并评估术后效果。方法2019年4月至2021年12月,四川大学华西口腔医院唇腭裂外科手术治疗单侧微小型唇裂患儿27例,男13例、女14例,年龄6个月至25[0.7(0.8,2.0)]岁。运用力矩平衡理论与技术重建微小型唇裂美学形态。拍摄术前及术后照片,测量人中偏斜角,唇高、侧唇宽、唇峰点红唇厚度、唇峰至面中线距离的不对称率(即健侧/患侧的比值减1的绝对值,值越小代表健患侧越对称)等指标来评估手术效果。结果与术前相比,27例微小型唇裂患儿术后的人中偏斜角(P<0.001)、唇高不对称率(P<0.001)、侧唇宽不对称率(P=0.019)、唇峰点红唇厚度不对称率(P=0.021)、唇峰至面中线距离不对称率(P=0.004)均小于术前,差异均有统计学意义(均P<0.05)。结论力矩平衡理论与技术重建微小型唇裂美学形态后,健患侧对称性较术前有了明显改善,为微小型唇裂的美学形态重建提供了新方案。Objective To use the torque balance theory and technology to reconstruct the aesthetic morphology of microform cleft lip and to evaluate its short-term postoperative effect.Methods A total of 27 patients with microform cleft lip were included in the study,and the aesthetic morphology of microform cleft lip was reconstructed by force balance theory and technology.There were 13 males and 14 females,ranged in age from 6 months to 25 yeare.The photometric two-dimensional indexes,including the philtrum oblique angle and asymmertry ratios(lip height,lip width,vertical distance from the white roll to the vermilion bottom at the Cupid′s bow point,and vertical distance from the Cupid′s bow points to facial midline),were employed to measure and evaluate the outcome.Results The philtrum oblique angle(P<0.001),asymmertry ratios of lip height(P<0.001),asymmertry ratios of lip width(P=0.019),asymmertry ratios of vertical distance from the white roll to the vermilion bottom at the Cupid′s bow point(P=0.021)and asymmertry ratios of vertical distance from the Cupid′s bow points to facial midline(P=0.004)all were smaller than before surgery.All of the indexs showed statistically significant difference(P<0.05).Conclusions After the reconstruction of the aesthetic morphology of microform cleft lip guided by force balance theory and technology,the symmetry is significantly improved.This method provides a new scheme for the aesthetic morphology reconstruction of microform cleft lip,and the main features include the theory of muscle torque balance of microform cleft lip correction;new invisible incision approach,and innovative development of philtrum column and lip beads reconstruction technology.
