健脾清热生肌方联合美沙拉嗪治疗湿热型溃疡性结肠炎的疗效及对炎症因子的影响

摘　　要：目的:探讨健脾清热生肌方联合美沙拉嗪治疗湿热型溃疡性结肠炎(UC)的临床疗效及对炎症因子PLR、NLR和CRP的影响。方法:选取2020年1月至2023年3月,本院收治的90例UC患者,按随机数字表法分为观察组和对照组,每组各45例。对照组给予美沙拉嗪治疗,观察组在对照组的基础上给予健脾清热生肌方灌肠治疗,疗程为6周。治疗结束后评估2组疗效、不良反应发生率、中医证候积分(TCMSS)等情况,分析疗效指数与PLR、NLR、CRP的相关性。结果:观察组总有效率显著高于对照组(97.78%VS 84.44%,P<0.05),2组不良反应发生率比较,差异无统计学意义(P>0.05)。治疗后TCMSS、改良Mayo评分、Geboes指数及PLR、NLR、CRP水平均降低,且观察组低于对照组(P<0.05);疗效指数与PLR、NLR、CRP均呈正相关(P<0.05)。结论:健脾清热生肌方联合美沙拉嗪口服治疗湿热型UC疗效确切,能有效恢复外周血PLR、NLR和CRP水平,减轻炎症反应,改善临床症状,值得临床推广应用。Objective To investigate the clinical efficacy of Jianpi Qingre Shengji Formula combined with mesalazine in the treatment of damp-heat type of ulcerative colitis(UC)and its effect on inflammatory factors of PLR,NLR and CRP.Methods From January 2020 to March 2023,total 90 patients with UC admitted to our hospital were selected and divided into the observation group and the control group according to the random number table method,with 45 cases in each group.The control group was given mesalazine treatment,and the observation group was given enema treatment of Jianpi Qingre Shengji Formula on the basis of the control group,for 6 weeks.After treatment,the efficacy,the incidence of adverse reactions and the TCM syndrome score(TCMSS)of both groups were evaluated,and the correlation between the efficacy index and PLR,NLR,CRP was analyzed.Results The total effective rate of the observation group was significantly higher than that of the control group(97.78%VS 84.44%,P<0.05),and there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).After treatment,the TCMSS,the modified Mayo score,Geboes index and the levels of PLR,NLR and CRP were decreased,which were lower in the observation group than in the control group(P<0.05).The efficacy index was positively correlated with PLR,NLR and CRP(P<0.05).Conclusion The oral treatment of Jianpi Qingre Shengji Formula combined with mesalazine is effective in treating damp-heat type of UC,which can effectively restore the levels of PLR,NLR and CRP in peripheral blood,reduce inflammatory response,improve clinical symptoms,and is worthy of clinical promotion and application.
